摘要
目的:评价口服单剂量预防性抗生素在经直肠前列腺穿刺活检术中的临床应用效果。方法:自2005年9月至2009年3月总共192例患者接收经直肠超声引导13点前列腺系统穿刺活检术。所有患者随机分为3组。A组患者62例,口服安慰剂(维生素C)0.2每天2次,共3天;B组患者64例,口服单剂量吉米沙星320mg/d,加甲硝唑0.4/d,1次;C组患者66例,口服吉米沙星320mg/d加甲硝唑0.8/d,共3天;所有患者均在活检后48小时进行尿培养,发热者再进行血培养检查。结果:非感染性并发症包括:血尿,血便,疼痛。感染性并发症包括:尿路感染和发热。A、B、C三组患者感染性并发症的发生率为11%(7/62),3%(2/64),3%(2/66)。可见,感染性并发症的发生率在A组明显高于B组和C组,差异具有显著性意义(P<0.01)。而B、C两组间感染性并发症的发生率相当,差异无显著性意义(P>0.05)。结论:恰当应用单剂量抗生素预防经直肠前列腺穿刺活检术中的感染是有效的,也是足够的。
Objective To evaluate the efficacy and safty of single -dose oral antibiotic prophylaxis in the prevention of postproeedure infections in patients undergoing transrectal prostate biopsy. Methods Between September 2005 march 2009, A total of 192 patients referred for 'abnormal digital rectal examination and/or prostate specific antigen 4ng/ml or greater underwent transrectal ultrasound guided systematie 13 cores prostate biopsy. The patients were randomized into three groups. Patients in group A (62) then received a placebo (Vit c) tablet twice a day for 3 days; those in group B (64) were given a single dose of Gemifioxacin (320mg/d) and metronidazole (0. 8g/d), while those in group C (66) were given the same combination, for 3 days. Urine cultures were obtained 48h after the biopsy and blood cultures only from patients who developed fever. Results Noninfective complications included were rectal bleeding , haematufia and pain. The infective complications included urinary tract infection and fever. There was no significant difference among the three groups in noinfective complications but the incidence of infective complications in group A was significantly higher than group B and C (P 〈0. 01 ), and there was no significant difference among group B and C in infective complications ( P 〉 0. 05) . Conclusion Single - dose oral antibiotic prophylaxis is a effective and safe approach to prevent infectious complications from transrectal prostate biopsy.
