环氧合酶-2及血管内皮生长因子对卵巢上皮性癌手术预后的影响 被引量:1

The effect of COX-2 and VEGF on epithelial ovarian cancer prognosis.

摘要 目的分析和评价环氧合酶-2(COX-2)及血管内皮生长因子(VEGF)对卵巢上皮性癌患者手术预后的影响。方法选取中国医科大学附属盛京医院1997年1月至2002年1月收住院行手术治疗并经术后病理证实的183例原发性卵巢上皮性癌病例,将可能与卵巢上皮性癌预后有关的常用临床病理因素和生物学因素(COX-2、VEGF)进行单因素分析。通过COX模型对单因素分析筛选出的因素进行多因素分析。结果共获得确切随访病例147例,3、5年生存率分别为45.71%、30.71%。将8个临床病理因素及生物学因素进行单因素分析,结果显示:绝经前后、手术病理分期、组织学分级、COX-2蛋白阳性表达、VEGF蛋白阳性表达与预后明显相关(均P<0.05),淋巴结转移、病理类型、化疗与预后无关(均P>0.05)。应用COX模型对单因素分析筛选出的5个因素进行多因素分析表明:COX-2蛋白阳性表达、手术病理分期、VEGF蛋白阳性表达、组织学分化程度,其相对危险度分别为2.825、2.292、2.051、1.873。COX-2蛋白阳性表达者死亡危险度是COX-2蛋白阴性表达者的2.825倍。结论COX-2蛋白阳性表达、手术病理分期、VEGF阳性表达、组织学分级是影响卵巢上皮性癌手术预后显著的因素;COX-2蛋白阳性表达为第1位因素。 Objective Analyze and evaluate the normal clinic-pathological characteristics (menopause, FIGO stage , histological grade and types, lymph node transmission, and histological type, chemotherapy) and biochemical factors ( COX-2 and VEGF) fur prognosis of epithelial ovarian cancer. Methods Analyze the clinic-pathological and biochemical factors which are possible affect ovarian cancer prognosis by univafiate analysis and multivariate analysis through COX proportional hazard regression model. Results 183 epithelial ovarian cancer cases were invested, 147 of which were followed-up successfully. The three-year survival rate was 48.71%. The five-year survival rate was 30. 71%. Analyzing the 6 clinic-pathological factors and 2 biochemical factors through univafiate analysis, the menopause, FIGO stage, histological grade, COX-2 status, VEGF status are associated with prognosis (P 〈 0. 05 ) , while lymph node transmission , histological type and chemotherapy are not associated ( P 〉 0. 05 ). These 5 factors are analyzed by Cox proportional hazard regression model analysis. The result is COX-2 status, FIGO stage, VEGF status, histological grade, are the most important factors for ovarian epithelial ovarian cancer prognosis. The death risk degree for COX-2 positive, FIGO stage, VEGF positive, and histological grade is 2. 825, 2. 292, 2.051 and 1. 873 respectively. Conclusion By Cox proportional hazard regression model analysis COX-2 status, FIGO stage, VEGF status, histological grade, are the important risk factors prognosis factors for epithelial cancer prognosis. COX-2 positive is the first risk factor for affecting epithelial ovarian cancer.
