2型糖尿病血管并发症与血脂和脂质比值的关系 被引量:8

Relationship between dyslipidemia and vascular complications with type 2 diabetes mellitus

摘要 目的探讨2型糖尿病血管并发症与血脂和脂质比值的的关系。方法选择2型糖尿病患者180例,分为糖尿病无血管并发症组60例(B组),糖尿病微血管并发症组60例(C组),糖尿病大血管并发症组60例(D组),并选择正常对照组100例(A组),检测各组血脂并计算脂质比值。结果B、C和D组与A组比较,总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平均明显升高,分别为TC(4.85±0.53)mmol/L、(5.24±0.90)mmol/L、(5.75±0.89)mmol/L vs(4.41±0.66)mmol/L,TG(1.93±1.26)mmol/L、(2.15±1.69)mmol/L、(2.02±1.16)mmol/L vs(1.04±0.34)mmol/L,LDL-C(2.68±0.43)mmol/L、(2.93±0.68)mmol/L、(3.44±0.76)mmol/L vs(2.41±0.47)mmol/L(P<0.01);高密度脂蛋白胆固醇(HDL-C)水平明显降低,(1.17±0.30)mmol/L、(1.17±0.24)mmol/L、(1.11±0.15)mmol/Lvs(1.37±0.19)mmol/L(P<0.01);TC/HDL-C、LDL-C/HDL-C和TG/HDL-C的对数转换值(lgTG/HDL-C)水平均明显升高,分别为TC/HDL-C 4.36±1.09、4.64±1.11、5.25±0.87 vs3.26±0.56,LDL-C/HDL-C2.45±0.75、2.60±0.70、3.13±0.64 vs 1.80±0.43,lgTG/HDL-C0.16±0.31、0.20±0.29、0.21±0.23 vs-0.14±0.17(P<0.01)。B、C和D组间TC、LDL-C水平依次增高(P<0.01或<0.05)。与B、C组比较,D组TC/HDL-C、LDL-C/HDL-C水平明显升高(P<0.01)。结论TC和LDL-C增高是2型糖尿病血管并发症的危险因素,而脂质比值TC/LDL-C、LDL-C/HDL-C增高在糖尿病大血管并发症的发生发展中起着重要作用。 Objective To explore the relationship between dyslipidemia and vascular complications with type 2 diabetes mellitus(T2DM). Methods A total of 180 patients with T2DM were enrolled in the study:60 patients without vascular complications (group B), 60 patients with microvascular complications (group C), 60 patients with macrovascular complications(group D), meanwhile 100 individuals served as normal controls(group A). Blood lipids were measured and lipids ratio was calculated. Results Compared with group A,the levels of serum total cholesterol (TC) ,triglycerides(TG), low density lipoprotein-cholesterol(LDL-C) in group B, C and D increased remarkably, TC (4.85±0.53) mmol/L,(5.24±0.90) mmol/L, (5.75±0.89) mmol/L vs (4.41±0.66) mmol/L,TG (1.93±1.26)mmol/L, (2.15 ± 1.69 ) mmol/L, ( 2.02 ± 1.16 ) mmol/L vs ( 1.04 ± 0.34) mmol/L, LDL C ( 2.68 ±0.43 ) mmol/L, (2.93±0.68) mmol/L, (3. 44 ± 0. 76) mmol/L vs (2.41±0.47) mmol/L(P 〈0.01),the level of high density lipoprotein-cholesterol (HDL-C) decreased significantly, (1.17 ± 0.30) mmol/L, (1.17 ± 0.24) mmol/L, (1.11 ± 0.15) mmol/L vs (1.37 ± 0.19) mmol/L( P 〈0.01 ), the levels of TC/HDL-C, LDL C/HDL-C and lgTG/HD-C increased obviously,TC/HDL-C 4.36±1.09,4.64±1.11,5.25±0.87 vs 3.26±0. 56, LDL C/HDL-C 2.45±0.75, 2.60±0.70,3.13±0.64 vs 1.80±0.43,lgTG/HDL-C 0.16±0. 31,0. 20±0. 29,0. 21±0. 23 vs -0.14±0.17 ( P 〈 0.01). The levels of TC and LDL-C successively increased in group B,C and D ( P 〈0.01 or 〈0.05). The levels of TC/HDL-C and LDL-C/HDL-C in group D were obviously higher than those of group B and group C ( P 〈0.01). Conclusion The increase of TC and LDL-C are the risk factors for vascular complications of T2DM, the increase of TC/LDL-C and LDL-C/HDL-C play an important role in the development of macrovascular complications of T2DM.
