摘要
目的检测急性肺损伤患者血清VEGF水平的变化,探讨血清VEGF水平、急性肺损伤评分、氧和指数与APACHEII评分的相关性。方法收集2006年7月~2007年10月我院ICU收治的急性肺损伤患者的临床资料,计算APACHEII评分、急性肺损伤评分和氧和指数。采用ELISA方法检测患者发病后第1天、第7天及健康对照组血清VEGF水平。结果急性肺损伤患者在发病早期(第1天)VEGF水平即明显升高(177.69±30.52pg/ml),与对照组(83.27±7.95pg/mL)比较,差异有统计学意义(t=-13.39,P<0.01)。生存组及死亡组患者在肺损伤早期VEGF均明显升高,两者比较差异无统计学意义,但随着病程进展,生存组第7天VEGF水平下降,与第1天比较,P<0.01。死亡组VEGF水平居高不下,与第1天比较P>0.05。VEGF水平与APACHEII评分呈正相关rp=0.386,P=0.014(P<0.05),氧和指数、急性肺损伤评分与APACHEII评分未显示相关性,P>0.05。结论急性肺损伤早期血清VEGF水平升高,动态监测血清VEGF水平可作为判断疾病严重程度指标之一。
[ Objectives ] To examine the variety of the serum VEGF levels in acute lung injury patients, and to investigate the relationship of VEGF levels, acute lung injury score and oxygenation index of patients with APACHEII score. [Methods] Enroll the clinical materials of acute lung injury patients who were hospitalized in ICU of our hospital from July 2006 to October 2007, and calculated APACHEII score, acute lung injury score and oxygenation index. Examine the VEGF levels in acute lung injury patients on thelst/7thday and control group VEGF levels by ELISA. [Results] The VEGF levels (177.69 ± 30.52pg/mL) are elevated in acute lung injury patients fall ill early stage(theist day), it is obviously higher than the control group (83.27± 7.95pg/mL), there has statistical difference (t =-13.39, P 〈0.01). Both the survival group and the death group, the VEGF levels are elevated in the 1st day, there has no statistical difference. But the VEGF levels decreased with the development of the disease in the survival group on the 7th day, there has statistical difference between the 7th day and the 1st day (P 〈0.01). In the death group, VEGF levels decreased unconspicuously on the 7th day, there has no statistical difference between the 7thday and thelst day (P 〉0.05). The VEGF levels has positive relation with APACHEII score (rp=0.386, P =0.014) (P 〈0.05), but acute lung injury score and oxygenation index of patients has no relation with APACHEII score (P 〉 0.05). [ Conclusion ] The VEGF levels are elevated in acute lung injury patients fall ill early stage and to dynamic monitor the serum VEGF levels can be used as an indicator of the severity of acute lung injury.
