摘要 内皮素(ET)尤其是内皮素-1(ET-1)被认为是内皮素家族中对心血管和肾脏作用最强大的内源性缩血管物质。ET通过强烈的收缩血管作用,导致内皮功能不良,动脉粥样硬化,诱发慢性肾脏病(chronic kidney disease,CKD)患者的心血管事件。另一方面,ET还可促进肾脏固有细胞增殖,介导炎症反应,刺激细胞外基质合成,从而加速肾脏纤维化。内皮素系统拮抗剂不仅能降低CKD患者的心血管事件风险,更重要的是有助于改善其肾功能,并降低蛋白尿,为肾脏病防治提供新思路。 Endothelin-1 ( ET-1 ), one of the endothelin family, is the most powerful endogenous vasoconstrictor and predominant isoform in the cardiovascular and renal systems. ET contributes to endothelial dysfunction and atherosclerosis, so it lends to cardiovascular accident. On the other hand, ET stimulates renal cells proliferation, inflammation and extracellular matrix synthesis. Through those, ET promotes renal fibrosis. Antagonist of ET system may not only reduce cardiovascular risk, but also may be of benefit in improving renal function and reducing proteinuria, which provide a new way for renal disease treatment and prevention.
