摘要
目的探讨在完全腹腔镜下,对胰体尾部肿瘤进行保留脾脏和脾血管的胰体尾切除术的可行性和安全性。方法2008年10月和12月对2例胰尾胰岛素瘤患者施行完全腹腔镜下保留脾脏的胰体尾切除术。结果2例手术均顺利完成,手术时间分别为120min和100min,术中出血为50ml和100ml,术后住院时间为5d和7d,术后无并发症产生,术后病理诊断均为胰尾胰岛细胞瘤。结论对胰尾部的良性病变行完全腹腔镜下保留脾脏的胰体尾切除术是微创、安全可行的,具有恢复快、并发症少等优点,值得进一步推广。
Objective To investigate the feasibility and safety of total laparoscopic distal pancreatectomy with preservation of the spleen and its blood vessel in treatment of pancreatic tumours. Methods From Oct. 2008 to Dec. 2008, 2 patients with islet cell tumour in the tail of the pancreas underwent total laparoscopic distal pancreatectomy with preservation of the spleen. Results Both of the operations were successful with the operative time of 120 min. and 100 min. respectively. And the blood loss were 50 ml and 100 mL The postoperative hospital stay were 5 d and 7 d. There were not any postoperative complications were found. The pathological diagnosis were islet cell tumour. Conclusion Total laparoscopic distal pancreatectomy with preservation of the spleen is minimal invasive. Safe and feasible for the management of benign pancreatic tail tumours. With the advantages of earlier recovery and fewer complications. So it is worth being spread.
