摘要
目的探讨皮质醇对脓毒症大鼠细胞因子的影响及可能的治疗作用。方法采用大鼠CLP脓毒症模型。将58只SD大鼠分为A、B、C、D、E组,A、C组于CLP模型后每12小时皮下注射氢化可的松(30mg·kg^(-1)·BW),B、D组于CLP模型后相同时间点皮下注射等量生理盐水,E组为非模型对照组。结果 A组3例死亡,B、C、D、E组无死亡。A组血浆皮质醇水平和TNF含量均显著高于B、C、D、E组(P<0.05)。A、B、C、D组腹腔液中TNF-α含量显著高于E组(P<0.05)。所有大鼠腹腔液中TNF-α、IL-6含量均显著高于血浆(P<0.05),各组间血浆和腹腔液中IL-6含量均无显著差异(P>0.05)。结论脓毒症时应用皮质醇激素治疗可使血浆及腹腔液中TNF-α含量升高,伴有动物死亡率增加。但对血浆及腹腔液中IL-6含量无显著影响。
Objective To investigate the effects of cortisol on the levels of TNFα and IL-6 in septic rats with CLP and the possible therapeutic effects of cortisol to sepsis. Methods Fifty-eight Sprague-DaMey rats were divided into 5 groups. The rats in groups A and C were injected subcutaneously with hydrocortison in an interval of 12 h after CLP model, while the rats in groups B and D received the injection of normal saline (NS). Those in group E underwent laparatomy without CLP and injection of NS. All the animals were killed at different points. Results The eortisol level in group A was significantly higher than in group C (P<0. 01). The cortisol level in plasma of groups B, D and E was undetectable. The average level of TNFα in plasma in group A was significantly higher in groups B, C, D, and E (P<0. 05). No significant differences in TNFα level in plasma were found between B, C, D and E groups (P>0. 05). TNFα in aseitic fluid was detectable in all groups and that in groups A,B,C,and D was significantly high-er than in group E (P<0. 05). TNFα level in ascitic fluid was significantly higher than in plasma in all groups (P<0. 01). IL-6 level was found in plasma and ascitic fluid between A, B, C, D, and E groups (P>0. 05), but the 1L-6 level in ascitic fluid was significantly higher than in plasma in all groups (P<0. 01). Conclusion Treatment to septic rats with eortisol significantly elevated TNFα level in plasma and ascitie fiuid. Animals sepsis was not significantly affected by the administration of high dose of cortisol. In view of cytokines, treatment for severe infection or sepsis with a high dose of cortisol may be harmful to the body.
