强离子隙等酸碱失衡诊断指标在创伤中的应用 被引量:4

摘要 目的探讨强离子隙(SIG)、阴离子隙(AG)等指标在创伤中的变化及其应用价值。方法对创伤性疾病患者入院后立即抽取动脉血,肝素抗凝,混匀。用丹麦雷度ABL555血气分析仪检测血气测定值,同时抽取静脉血用美国库尔特贝克曼LX20生化分析仪检测肌酐(Cr)、磷及清蛋白(ALB)。结果创伤性疾病组与对照组相比较SIG、AG、潜在HCO3-等5项差异有统计学意义(P<0.002)。在创伤性疾病组中AG、潜在HCO3-、AGcorr、与SIG相关性好。创伤死亡组与创伤非死亡组的平均数比值以SIG、AG、Cr最高。结论在创伤性疾病组中所检测项目中,以SIG、AG的应用价值较高。即将死亡的创伤病例SIG均>4mmol/L,明显升高。 Objective To Investigate the application value of strong ion gap(SIG) ,anion gap(AG) and other four indicators in the trauma disease. Methods Blood was taken from trauma patients admitted to hospital immediately,with the heparin anticoagulation , misce bene. Blood gas was measured. Creatinine ( Cr), phosphorus and albumin (ALB) was measured simultaneously. Results Betwean traumatic group and control group, there was signifrcant difference in SIG, AG, potential HCO3^- , and other 5 item ( P 〈 0. 002). In traumatic diseases group, AG, potential HCO3^- , AG had relation with with SIG. Average proportionality of SIG, AG, Cr were the highest items in traumatic death groups and non - death group. Conclusion SIG and AG has higher application value in the trauma of disease groups. SIG in cases to be death is 〉4mmol/L.
