摘要
目的:探讨多层螺旋CT双期脑血管成像对颈内动脉海绵窦瘘(ICCF)的诊断价值。方法:10例临床怀疑或确诊颈内动脉海绵窦瘘(ICCF)患者,均行颅脑CTA双期增强扫描,其中7例为术前检查,3例为介入栓塞术后复查,5例同期行DSA检查。采用容积再现(VR)、最大密度投影(MIP)、多平面重组(MPR)等技术处理图像。结果:7例术前CTA动脉期示患侧海绵窦早显扩大,同侧眼上静脉扩张纡曲,其中3例可见瘘口,3例同侧皮层静脉扩张,5例见岩上和岩下窦扩张,4例对侧海绵窦及眼上静脉显影扩大,3例患者有颅骨或颅底骨折。3例术后复查CTA示患侧海绵窦及眼上静脉无早显扩张,与DSA所见一致。结论:16层CT双期脑血管成像对颈内动脉海绵窦瘘的诊断及介入栓塞术后疗效观察有重要价值。
Objective:To discuss the value of dual-phase multi-slice spiral CT angiography (MSCTA) in the diagnosis and follow-up of carotid-cavernous fistula (ICCF). Methods:10 cases with suspected or proved ICCF had dual-phase MSC- TA performed,including 7 patients before surgery and 3 patients with follow-up examination after intra-vascular embolization; 5 patients had DSA simultaneously. Post-processing techniques including volume rendering (VR), maximum intensity projection (MIP),multi-planar reformation (MPR) were performed. Results: Early visualization and enlargement of the affected cavernous sinuses communicating with homo-lateral dilated supraobital vein were found in the arterial phase of MSC- TA in all 7 patients before surgery. Orificium fistulae (3 cases) ,homo-lateral cortex vein dilatation (3 cases) superior/inferior petrosal sinus dilataion (5 cases), enlargement of contra-lateral cavernous sinuses communicating with dilated supraobital vein (4 cases) and skull or skull base fractures (3 cases) were revealed. Of the 3 post-surgery patients,no evidence of early assessment of the affected enlarged cavernous sinuses communicating with dilated supraobital vein was showed. Conclusion:Dual-phase MSCTA with 16-slice spiral CT is of great value in the diagnosis and follow-up examination of carotid- cavernous fistula.
