摘要 目的检测并比较神经细胞黏连分子(NCAM/CD56)与常用神经内分泌标记物嗜铬蛋白(CgA)和突触素(Syn)对小细胞肺癌(SCLC)的诊断价值。方法采用MaxVision免疫组化法对125例SCLC存档病例(包括手术标本和活检标本)行CD56、CgA和Syn检测,以χ2检验分析表达结果。结果小细胞肺癌CD56阳性率为90.4%(113/125),肿瘤组织弥漫(+),阳性强度多为中~强阳性(65/125);CgA阳性率为56.8%(71/125),阳性着色位于胞质内,颗粒状,多为弱(+),肿瘤组织多呈灶性(+);Syn阳性率为82.4%(103/125),多为弥漫弱(+)(891125),部分病例呈灶状(+)。CD56和Syn两者在阳性率上差异不显著(P〉0.05),但两者阳性率均高于CgA,且差异显著(P〈0.05)。结论作为小细胞肺癌的辅助诊断标记物,CD56和Syn阳性表达率高,是可靠的神经内分泌标记物,其中CD56为首选。 Objective To investigate the expression of neural cell adhesion molecule (NCAM/CD56), chromogranin A (CgA) and synaptophysin (Syn) in small cell lung carcinoma (SCLC) and to assess their diagnostic value. Methods 125 SCLC cases had been collected from the archive of the Department of Pathology in PLA General Hospital during the last 4 years (both surgical specimens and biopsies were included). All cases were stained for CD56, CgA and Syn by Max-Vision immunohistochemical technique. The data were analyzed by Chi-square test. Results 90.4%(113/125)of SCLC cases were stained positively for CD56. Generally, the stain was diffuse and most eases (65/125) were moderately to intensely. 56.8% (71/125)of the eases showed weakly and focally pesitive for CgA. The positive percentage for Syn was 82.4% (103/125). The reaction was diffuse and weak in most cases (89/125) with a small proportion of them being focally stained. The positive percentages for CD56 and Syn were insignificantly different ( P 〉 0.05), but both of them were significantly higher than that of CgA ( P 〈 0.05). Conclusion As auxihary molecular markers for the diagnosis of SCLC, CD56 and Syn, showing a positive stain in most SCLC cases, are confirmed to be reliable neuroendocrine markers. With its stable expression in SCLC, CD56 was proved to be the optimal molecular marker for the confirmation of SCLC.
