摘要
为观察安装起搏器术后患者起搏器功能,采用24小时动态心电图(Holter)对20例安装心脏起搏器患者进行随访,结果发现8例出现起搏器感知障碍,其中1例为感知过度,其余7例为感知不良。起搏器感知过度的1例无症状,起搏器感知不良的7例中,2例感知障碍频繁出现,病人有症状,需要换起搏器,另外5例无症状,为短暂起搏器感知失效,常规心电图示感知正常。对该5例中2例进行2~4次随访,其感知失效减少或消失。由此认为,对于发现有症状和无症状的一过性起搏及感知功能失效,Holter比其它起搏检查技术更具优越性,有少数安装起搏器患者可出现原因不明的短暂感知不良而无症状,对于这类病人可定期复查,不需处理。
For follow-up of the function of pacemakers,20 patients received 24-hour ambulatory electrocardiogram(Holter)monitoring.The results showed pacemaker sensing dysfunction was found in 8 patients.Of these 8 patients with pacemaker sensing dysfunction,1 patient was oversensing and the other 7 patients were undersensing.The patients with oversensing had no any symptoms.Of these with undersensing,2 had frequent sensing failure in conjunction with dizziness needing to change pacemaker and 5 had transient sensing failure with no symptom.In two patient receiving Holter monitoring for 2~4 times,sensing failures were reduced in the subsequent monitoring.These findings suggest that Holter monitoring be superior to other techniques in terms of finding both symptomatic and asymptomatic dysfunction of pacemakdr pacing or sensing.For these patients who have transient sensing dysfunction without symptoms,Holter monitoring at intervals was allow and no treatments were needed.
