磁共振弥散张量成像对颈髓损伤的临床诊断价值 被引量:5

摘要 目的探讨磁共振弥散张量成像(diffusion tensor imaging,DTI)对颈髓损伤的临床价值。方法选择42例健康志愿者作为健康对照组和54例脊髓型颈椎病患者进行颈椎常规MRI及弥散张量成像,分别测量各组表观弥散系数(apparent diffusion coefficient,ADC)、部分各向异性(fractional anisotropy,FA)值,并显示其弥散张量纤维束(diffusion tensor tracking,DTT)。结果正常组的平均ADC值为(830.34±215.86)×106mm2/s和平均FA值为(536.03±40.00)×10-3。颈髓慢性损伤患者平均A D C值为(1107.60±47.55)×106mm2/s,较正常组升高,有统计学意义(P<0.01),平均FA值为(425.91x±59.48)×10-3,较正常组下降,有统计学意义(P<0.01)。结论磁共振弥散张量成像较常规MRI成像能早期显示脊髓的损伤,ADC值、FA值、DTT图是检测早期脊髓损伤微观结构的敏感指标。 Objective To investigate the MR diffusion tensor imaging on the clinical application of cervical cord injury. Methods Conventional MRI scans of cervical and diffusion tensor imaging were performed in 54 patients with cervical spondylotic myelopathy and 42 cases of heakhy volunteers as a healthy control group, apparent diffusion coefficient and fractional anisotropy were measured, diffusion tensor tracking was showed in each group. Results The average ADC value is(830.34 ± 215.86) ×10^6mm2/s. The average FA value is (fractional anisotropy, FA)in control group; in contrast the average ADC value is(1107.60 ± 47.55)×10^6mm2/s, higher than the control group , which showed statistical significant difference(P〈0.01 ). The average FA value is(425.91x ± 59.48) ×10^3,lower than the control group which showed statistical significant difference (P〈0.01) Conclusion MR diffusion tensor imaging can show the early injury in the spinal cord better than conventional MR/imaging. ADC values, FA values, DTT is a sensitive indicator of the early detection of micro-structure of spinal cord injury .
