摘要
目的了解冠心病患者戒烟后不同时期的血脂、血液流变学及颈动脉内膜中层厚度的变化。方法90例经冠脉造影确诊为冠状动脉粥样硬化性心脏病(以下简称冠心病)的并合吸烟史男性患者,分为吸烟组、戒烟组,每组45例。分别在戒烟前、戒烟后3个月及1年后对血脂、血液流变学指标及颈动脉内膜中层厚度进行组间及组内自身对照比较。结果戒烟3个月后血脂开始变化,1年后较戒烟前及吸烟对照组有明显改善,差异有显著性(P<0.05)。1年后血液流变学大部分指标较治疗前及吸烟对照组显著改善(P<0.05)。颈动脉内膜中层厚度戒烟3个月后较吸烟组未见有明显变化(0.896±0.213vs0.918±0.157,P>0.05),但12个月后该指标(0.806±0.135)较戒烟前(1.31±0.361)及吸烟对照组(0.854±0.162)显著改善(P<0.05)。结论戒烟对冠心病患者血脂,尤其对高密度脂蛋白胆固醇及血液流变学改善呈持续有益作用,较短时间戒烟不能改善颈动脉内膜厚度,但戒烟1年后颈动脉内膜厚度有逆转性改变。
Objective To evaluate the effects of smoking cessation on blood lipids, hemorheology and thickness of carotid artery intima media in patients with coronary heart disease. Methods Ninety patients with coronary lesions were randomized into two groups: smoking group and smoking cessation group(45 patients in each group). Evaluation of blood lipids,hemorheology and thickness of carotid artery intima media was performed with eehocardiography and hemodynamie studies before, three months and twelve months after smoking cessation. Results Blood lipid changes were observed three months after smoking cessation. Significant decrease of TC and LDL - TC and increase of HDL - TC were revealed with blood lipid analysis after twelve months. In addition, hemodynamie studies also showed significant improvement in smoking cessation group, compared with smoking control group. Thickness of carotid artery intima media, measured with eehoeardiography, did not change three months (0. 896 ±0. 213 vs 0. 918 ±0. 157 ,P 〉0. 05)later, but showed a significant decrease (0. 806 ±0. 135 vs 0. 854 ±0. 162,P 〈0. 05) compared with the smoking control group one year later. Conclusion This study suggests that smoking cessation improves blood lipid and hemorheology, especially HDL- TC. Thickness of carotid artery intima media decreased significantly in patients with coronary heart disease one year but not a short - time after smoking cessation.
