摘要
目的探讨综合性医院艾滋病(AIDS)的流行特点。方法收集1997年8月-2008年12月在深圳市人民医院确诊的人类免疫缺陷病毒感染者/艾滋病患者(HIV/AIDS)病历资料进行描述性流行病学分析。结果2007年AIDS发病率为0.938/10万,比最低年份(1999年)增加11.9倍,平均增幅278.9%,经Cox-stuarrt检验,发病呈上升趋势(z=-3.532,n=5,P<0.01)。60例(83.3%)为AIDS现患者(其中2例是住院前通过接受免费抗HIV检测发现),12例(18.1%)HIV感染者中妇产科7例和外科2例HIV常规初筛(75%)及内科3例(25%)查因发现。曾在私立医院诊治有34例(62.5%),35例(64.8%)发病至住院多见于1-3月,尚有10例是第2次及以上住院确诊。2008年入院当天初筛率为88.9%,比2008年之前增加43.3%(P<0.05)。结论对高危险人群通过社区参与,确实地开展宣传教育及AIDS免费咨询及检测。继续提高诊断率。
Objective To explore epiderniologic characteristics of hospitalized patients with acquired immune deficiency syndrome （AIDS）/HIV infection in general hospital. Methods The medical record of in-patients with definite diagnosis of AIDS/HIV infection in the People＇ s Hospital of Shenzhen from 1997 to Decernher, 2008 were retrieved and analyzed with descriptive epidemiology. Results A total of 60 cases of AIDS and 12 cases of HIV infection were retrieved. The first AIDS inpatient of the hospital was diagnosed in 1999. There was an increase trend for the numner of patients with AIDS/ HIV infection in the hospital based on Cox-stuanrt test （ z = - 3.532, n = 5, P 〈 0. 01 ）. Among the 60 AIDS cases, 34 （62. 5% ） had history of visting private hospitals and 35 （64. 8% ） had hospitalization of 1 -3 months after the occurrence of AIDS symptoms. Among the 12 HIV infection cases, 9 （ 75.0% ） were diagnosed by routine screen for HIV and 3 （25.0%） were diagnosed through examination during hospitalization. The HIV screen rate at the day of hospitalization increased to 88. 9% in 2008 with a 43.3% increment than before （ P 〈 0. 05）. Conclusion Community based health education and voluntary consultation and test free of charge should be promoted to increase diagnosis of AIDS/HIV infection among high risk population.
