应用免疫胶体金银染色法检测大肠息肉癌胚抗原

Detection of Carcinoembryonic Antigen in Polyps of Large Bowel by Immunogold Silver Staining

摘要 为探讨癌胚抗原(CEA)对判断大肠息肉癌变危险性的价值,应用CEA、McAb借助免疫胶体金银染色法(IGSS)对大肠正常粘膜、息肉及癌组织中CEA进行检测,结果显示:多数大肠正常粘膜、炎性息肉及幼年性息肉CEA呈现阴性,仅少数CEA呈弱阳性,染色见于腺体细胞腔面,与上述各组比较,大肠增生性息肉和腺瘤组织CEA阳性率和染色程度均增高,随着腺瘤组织异型程度加重,其免疫组化改变及CEA分布更类似于大肠癌,结果提示CEA可作为监测大肠息肉癌变的标志物。IGSS染色法操作简便、敏感性高。 To study the value of carcinoembryonic antigen (CEA) in assessing the risk of malignancy of large bowel polyps, CEA in normal mucosa, polyps and carcinomas of large bowel were detected using CEA monoclonal antibody and stained by immunogold silver staining (IGSS). The results showed that in most normal mucosa, inflammatory polyps and juvenile polyps, CEA were negative; in a small number of them, weak positive staining were localized mainly to the apical cytoplasm of the columnar cells lining the upper levels of the crypts and mucosa surface. As compared with each group mentioned above, the positive rate and intensity of CEA in hyperplastic polyps and adenomas were high. As the degree of dysplasia of adenomas increased, the immunohistochemical charges mentioned above and abnormal distribution of CEA in the adenomas were more similar to those in the carcinomas. Our results showed that CEA could be used as one of the markers monitoring the malignant change in large bowel polyps. IGSS is an easy method with high sensitivity and worth using widely.
