Coopdech封堵支气管导管用于单肺通气手术的可行性 被引量:4

Feasibility study of Coopdech bronchial blocker tube in patients with one-lung ventilation

摘要 目的评价Coopdech封堵支气管导管用于单肺通气(OLV)手术患者的效果。方法选取需行OLV手术的患者40例,随机分为双腔支气管导管组(A组)和封堵支气管导管组(B组),每组各20例。比较两组患者插管时间、定位时间、术中肺萎陷情况、插管前后心率和血压变化及OLV时动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、脉搏血氧饱和度(SpO2)、pH值和气道压力(PAW)的变化。结果B组插管时间、定位时间分别为(1.03±0.26)、(1.00±0.15)min,低于A组的(2.77±0.83)、(4.09±0.77)min;插管后A组平均动脉压、心率升高;OLV后30、60min时B组PAW分别为(18.6±2.6)、(18.2±2.7)mbar,低于A组的(23.9±3.3)、(23.5±3.1)mbar,同时两组PaO2、SpO2、PaCO2、pH值比较差异无统计学意义。结论Coopdech封堵支气管导管能够为临床手术提供有效的OLV。 Objective To evaluate the feasibility of Coopdech bronchial blocker tube in patients with one-lung ventilation (OLV). Methods Forty ASA Ⅰ - Ⅱ patients with OLV were divided into two groups (20 eases each): group A with double tube catheter, group B with Coopdech bronchial blocker tube. The following variables were recorded: time of intubation tube and location, lung collapse, Bp, heart rate (HR), and partial pressure of carbon dioxide in artery(PaCO2), arterial oxygen tension (PaO2), pulse oxygen saturation (SpO2), pH and airway pressure (PAW) at 30, 60 min after OLV. Results The time of intubation tube and location in group B [ ( 1.03 ± 0.26), ( 1.00 ± 0.15 ) min] were lower than those in group A [ (2.77 ± 0.83 ), (4.09 ± 0.77 )min ]. There was no statistical difference in lung collapse between two groups. In group A, Bp and HR increased significantly after intubation compared with the baseline before intubation (P 〈 0.05 ). PAW was significantly lower at 30 min and 60 min after OLV in group B [ ( 18.6 ± 2.6 ) ,( 18.2± 2.7) mbar] than that in group A [ (23.9 ± 3.3 ), (23.5 ± 3.1 ) mbar]. There was no significant change in PaO2, PaCO2, SpO2 and pH. Conclusion It is feasible to use the Coopdech bronchial blocker tube to attain rapid and secure lung isolation for OLV.
