摘要
目的:通过对比Frank Ⅲ功能调节器和前方牵引矫治乳牙列反的疗效,总结出两种结果的共性与差异,为临床提供参考依据。方法:我科2004～2007年接诊乳牙列前牙反病例13例,随机分为功能矫治组和矫形矫治组,前者6例,后者7例。前者应用Frankl Ⅲ功能调节器治疗,后者采用口内全包埋式活动矫治器配合成品前方牵引器矫治,以X线头影测量数值评价疗效。结果:矫治结束后,矫形矫治组上颌骨明显前移(P<0.05),功能矫治组无统计学意义;矫治结束一年后,功能矫治组上颌骨向前生长明显优于矫形矫治组(P<0.05);两组对下颌骨的抑制作用无显著性差异。矫治后两组上前牙角度明显唇倾(P<0.001),矫治结束一年后继续唇向倾斜(P<0.05),增大量两组无显著性差异;矫治后下前牙角度在矫形矫治组无明显舌倾,功能矫治组下前牙明显舌倾(P<0.05),矫治结束一年后两组下前牙自主唇倾,去代偿,功能矫治组具有统计学意义(P<0.001)。结论:乳牙列反应尽早得到矫治,无论采取矫形力矫治或功能性矫治方法。功能调节器矫治主要以前牙唇倾度改变为主,随着前牙反的解除,上颌骨被诱导产生矢状向自主生长,对颌骨产生有利的影响。矫形力矫治以上颌骨前移、上前牙唇倾改变为主,效果较明显。
Objective To compare the effect of maxillary protraction and FRIII function regulator in the treatment of anterior cross-bite in deciduous dentition,so as to provide information for clinical treatment.Methods Thirteen anterior cross-bite patients were collected in Baoji Stomatological Hospital from 2004 to 2007.Patients were divided into two treatment groups,one group were treated with facemask orthopedic maxillary protraction by removable appliance,and the other group were treated with FRIII function regulator.Cephalmetric film was taken for each patient before and after treatment.Results In facemask group,maxillary moved forward more pronounced after treatment（P〈0.05）.At one year post-treatment,in FRIII group,the forward growth of maxillary were pronounced（P〈0.05）.No skeletal changes were observed on mandibular in two groups during the treatment and the post-treatment.In two groups,maxillary anterior teeth were procline after treatment（P〈0.001）.In FRIII group,mandibular anterior teeth were retrocline after treatment（P〈 0.05）.At one year post-treatment,in FRIII group,mandibular anterior teeth were procline significantly（P 〈0.001）.Conclusion The anterior cross-bite in deciduous dentition should be treated as soon as possible,either by maxillary protraction or by FRIII function regulator.FRIII function regulator treat it through the movement of anterior teeth,get rid of occlusion lock,and have a favorable growth of maxillary.Maxillary protraction correct deciduous cross-bite through dental and skeletal effect,and promote the growth of maxillary.
