美托洛尔对高血压患者非心脏手术围手术期的心脏保护

摘要 【目的】探讨美托洛尔对非心脏手术围手术期高血压痛患者脑利钠肽(BNP)和肌钙蛋白I(cTnI)的影响及其作用机制。【方法】220例非心脏手术高血压患者,110例预防性应用美托洛尔(实验组),另110例使用安慰剂(对照组),测定两组的心率、血浆BNP、cTnI、血糖和血脂,观察其心肌缺血发生率和心血管事件发生率。【结果】实验组患者拔管后的心率、收缩压、心肌缺血发生率、BNP、cTnI和心血管事件发生率显著低于对照组(P〈0.05)。【结论】预防性应用美托洛尔可有效减少非心脏手术围手术期高血压病患者发生心肌缺血、心肌损伤的风险,降低心血管事件的发生率。 [Objective]To investigate the effect of perioperative metoprolol on brain natriureuc peptide (BNP) and cardiac troponin I (cTnI), and evaluate its mechanism in hypertensive patients undergoing non-cardiac surgery. [Methods] Heart rate, plasma BNP, eTnl, fasting blood glucose and blood lipid were measured in 110 metoprolol-treated hypertensive patients undergoing non-cardiac surgery (experimental group) and 110 untreated patients (control group). [Results] Compared with the control group, heart rate, systolic pressure, the occurrence of myocardial ischemia, BNP, cTnI and the incidence of cardiac events were significantly decreased ( P 〈0.05). [Conclusion] Perioperative metoprolol may effectively decrease the occurrence of cardiac events via improving myocardial ischemia and protecting myocardial injury.
