摘要
作者报告1986年3月至1995年12月对337例食管癌的治疗。其中手术175例(51.9%),切除癌肿165例,手术切除率94.3%。手术死亡率1.8%(3/165)。术后发生胸部吻合口瘘3例,颈部吻合口瘘4例,吻合口瘘发生率4.2%(7/165)。本组随访率为91.2%。1年、3年、5年生存率分别为73.6%、42.5%、31.9%。本组中晚期食管癌较多,采取以手术为主辅以术前放疗和术后化疗以及免疫治疗等措施,减少了复发或转移,确实能延长生存期,从而提高了生存率。作者认为,对于中上段食管癌应行全胸段食管切除颈部食管胃吻合术。
Out of 337 inpatients with esophagus carcinoma,175 cases were treated by surgery.Among them,165 cases (94.3%) underwent radical esophagestomy.The motality rate for operation was 1.8%(3/165).The incidence of anastomotic leakage was 4.2%(7/165) (3 cases in chest,4 cases in neck).91.2% of patients were followed up to 1,3,5 years,the surval rate was 73.6%,42.5% and 31.9%,respectively.Besides surgery,29 cases received preoperative radiothrapy,53 cases had postoperative chemotherapy and 4 cases had postoperative radiotherapy.The data provied evidence that the combinative treatment could prolong the survived time,reduce the relapus rate or metastasis of tumor and increase the survived rate.Becanse of the biologic speciality of supramiddle esophagus carcinoma,the thoracic esophagectomy and esophagogastrostomy in neck should be necessary.
