摘要
目的了解高血压左室肥厚患者ＱＴｃ离散度（ＱＴｃｄ）的变化及其与左室肥厚的关系，同时观察依那普利治疗后的影响。方法测定２６例健康体检者（对照组），２８例高血压患者（ＥＨ组）及３０例高血压伴左室肥厚患者（ＬＶＨ组）依那普利治疗前后ＱＴｃｄ、左室重量指数（ＬＶＭＩ）及血压的变化。结果ＬＶＨ组ＱＴｃｄ（８１．５±２９．４ｍｓ）明显高于对照组（５１．３±２１．７ｍｓ）及ＥＨ组（５４．２±１９．２ｍｓ），且与ＬＶＭＩ呈正相关（ｒ＝０．７５４６，Ｐ＜０．００１），与血压不相关（ｒ＝０．２１４５，Ｐ＞０．０５）；经依那普利治疗后ＬＶＭＩ（１２３．７±２５．３ｇ／ｍ２）及ＱＴｃｄ（５８．５±２９．５ｍｓ）较治疗前（１４６．６±１７．５ｇ／ｍ２，８１．５±２９．３８ｍｓ）明显下降。结论高血压左室肥厚患者ＱＴｃｄ增高，且与左室肥厚程度相关；依那普利治疗可以逆转左室肥厚，降低ＱＴｃｄ。
Objective to study the changes of Q T interval dispersion(QTcd),the relationship between QTcd and left ventricalar hypertrophy(LVH) and the effects of enalapril treatment in hypertension patients with left ventricular hypertrophy \ Methods The QTcd and left ventricular mass index(LVMI) were measured in 26 control subjects,28 patients with essential hypertension and 30 patients with hypertensive left ventricular hypertrophy before and after a 6 month enalapril treatment Results (1)QTcd of the patients with hypertensive left ventricular hypertrophy(81 5±29 4 ms) was higher than that of simple hypertensive patients (54 2±19 2 ms) and control subjects(51 3±21 7 ms);was positively correlated to LVMI(r=0 7546,P<0 001) but not to blood pressure (r=0 2145,P>0 05) \ (2)Both QTcd and LVMI of patients with hypertensive left ventricular hypertrophy were decreased after 6 month enalapril treatment(58 5±29 48 vs 81 5±29 4 ms;123 7±25 3 vs 146 6±17 5 g/m 2) \ Conclusion\ QTcd of patients with hypertensive left ventricular hypertrophy increase and enalapril treatment can decrease QTcd and regress left ventricular hypertrophy
