摘要
目的研究不同剂量促红细胞生成素(EP0)对幼兔心肌缺血再灌注损伤的保护作用。方法32只幼兔随机分成对照组(A组)、3 000 U/kg EP0预处理组(B组)、5 000 U/kg EP0预处理组(C组)和7 000 U/kg EP0预处理组(D组),每组8只。B、C、D组均提前24 h腹腔注射EPO,A组腹腔注射2 ml生理盐水。建立Langendorff幼兔离体心脏灌注模型,测定缺血前和缺血再灌注120 min时的冠状动脉流量、心排血量、左心室收缩压和左心室舒张期末压。收集缺血再灌注60 min时的冠脉流液2 ml,检测肌酸激酶和乳酸脱氢酶的含量。取缺血再灌注后心肌,测定心肌含水量、心肌丙二醛含量、心肌三磷酸腺苷及磷酸肌酸含量;测定缺血前和缺血再灌注120 min时心肌细胞线粒体Na+,K+-ATP酶和Ca2+-ATP酶的活性。结果缺血再灌注后B、C、D组心功能恢复明显优于A组(P<0.05)。B、C、D组的心肌含水量及心肌乳酸脱氢酶、肌酸激酶、丙二醛含量均显著低于A组(P<0.05),B、C、D组心肌三磷酸腺苷、磷酸肌酸含量及心肌细胞线粒体Na+,K+-ATP酶和Ca2+-ATP酶的活性均显著高于A组(P<0.05)。C、D两组间比较,各项生化指标及心功能恢复均无显著性差异,但C、D组的心肌保护作用明显优于B组(P<0.05)。结论EPO预处理可显著降低未成熟心肌缺血再灌注损伤,具有良好的心肌保护作用。
Objective To study the protective effect of erythropoietin (EPO) on myocardial ischemia-reperfusion (I/R) injury in immature rabbits. Methods Thirty-two immature rabbits were randomly divided into 4 groups (n =8 each group). The rabbits were intraperiloneally injected with 2 ml of normal saline in group A, 3 000 U/kg of EPO in group B, 5 000 U/kg of EPO in group C, and 7 000 U/kg of EPO in group D 24 h before I/R, respectively. The Langendorff isolated perfused heart model was established, and the coronary flow, cardiac output, left ventricular systolic pressure, and left ventricular end-diastolic pressure before ischemia and 120 minutes after repeffusion were determined. The contents of creatine kinase and lactate dehydrogenase in coronary blood flow were measured 60 minutes after reperfusion. The myocardial water content and the contents of malondialdehyde,adenosine triphosphate,and creatine phosphate in the myoeardium were detected 120 minutes after reperfusion. The activities of Na^+, K^+-ATPase and Ca^2+-ATPase in myocardial mitoehondria were detected before ischemia and 120 minutes after repeffusion. Results The recovery of cardiac function in groups B,C,and D were better than that in group A (P 〈 0.05 ). Compared with group A, the myocardial water content and the contents of lactate dehydrogenase, creatine kinase, and malondialdehyde in the myocardium were significantly lower and the contents of adenosine triphosphate and ereatine phosphate and the activities of Na^+, K^+- ATPase and Ca^2+-ATPse in myocardial mitochondria significantly higher in groups B, C, and D (all P 〈 0.05 ). There was no significant difference in the biochemical indexes and the recovery of cardiac function between groups C and D, but the protective effect of EPO agaisnt myocardial I/R injury in groups C and D was better than that in group B (P 〈 0.05 ).
