摘要
目的探索和分析军人眼外伤的临床特点、救治现状和预后。方法对15家部队三级甲等医院2001-2005年716例军人眼外伤住院病例外伤分类及临床救治情况进行统计学分析。结果772只外伤眼中,机械性和非机械性眼球外伤分别占86.14%和7.51%。机械性眼球外伤中闭合伤占74.44%,以顿挫伤为主;开放伤占25.56%,以球内异物伤和穿通伤为主。眼球开放伤一期缝合救治71.82%于伤后24h完成,10.00%滞留超过2d;中心医院一期救治85.00%在伤后24h内完成,5.00%转送总医院行二次缝合。外伤性晶状体异常176眼(22.80%);视网膜脱离112眼(14.51%),其中70.79%行玻璃体切除术。出院统计眼球缺如率为2.85%,无光感率为1.17%,单眼盲达18.13%(眼球缺如计算在内)。结论顿挫伤是军人眼外伤的主要类别。基层医疗单位对严重眼外伤的转送仍不够及时,有必要加强和完善三级救治体系。
Objective To investigate and analyze the clinical characteristics of eye injury in military persons and its treatment and care as well as prognosis. Methods Eye injury data about 716 military inpatients obtained from 15 PLA general hospitals during 2001-2005 were statistically analyzed. Results Of the 772 injured eyes, mechanical globe injuries accounted for 86.14% and nonmechanical injuries accounted for 7.51%. Close globe mechanical injuries with contusion accounted for 74.44% and open globe mechanical injuries with intraocular foreign body and penetration accounted for 25.56%. Primary repair was performed within 24 hours after trauma in 71.82% patients and the repair was delayed in 10.00% patients for more than 2 days. In the central hospitals, primary repair was performed in 85.00% patients within 24 hours following trauma and 5.00% of the patients were transferred to general hospitals for a second suture. A total of 176 eyes (22.80%) had traumatic lens and 112 eyes (14.51%) had retinal detachment, of which 70.79% underwent vitrectomy. At discharge, 2.85% of injured eyeballs were removed, 1.17% had no light perception and 18.13% had monocular blindness including the removed eyes. Conclusion Contusion is the most frequent cause for eye injury in military persons. Since the grass-root PLA hospitals cannot promptly transport the patients with severe eye injury, it is necessary to strengthen and perfect the tri-grade treatment and care system.
