The Effect of Ligustrazine Hydrochloride on IL-6,IL-8 and Neutrophil after Cardiopulmonary Bypass in Infants with Congenital Heart Disease

摘要 目的:探讨盐酸川芎嗪对婴幼儿先天性心脏病体外循环(CPB)术后细胞因子IL-6、IL-8及中性粒细胞的影响。方法:40例经CPB行先天性心脏病手术的患儿随机分为观察组20例,对照组20例。观察组术前1 h静脉给予川芎嗪(5 mg/kg),对照组给予同等剂量生理盐水。分别在气管插管后10 min(T1)、转流后5 min(T2)、手术结束时(T3)、手术后2 h(T4)、手术后24 h(T5)采集动脉血3 mL,离心取血浆,通过酶联免疫吸附法(ELISA)检测致炎细胞因子IL-6、IL-8血浆浓度,同时对每一时点中性粒细胞计数。结果:两组IL-6在手术结束时(T3)、术后2 h(T4),IL-8在手术结束时(T3)升高显著(P<0.05),而对照组升高更显著(P<0.01),在手术后24 h(T5)均下降并接近正常,但IL-6对照组仍比观察组偏高;中性粒细胞CPB术后升高,但两组间差异无统计学意义。结论:术前应用盐酸川芎嗪对婴幼儿先天性心脏病CPB术后致炎细胞因子IL-6、IL-8有抑制作用。 Objective: To study the effect of Ligustrazine Hydrochloride on IL-6, IL-8 and neutrophil after cardiopulmonary bypass (CPB). Methods: Forty infants with congenital heart disease were randomly divided into two groups: 20 cases in the observing group and 20 cases in the control group. With the ELISA, the plasma concentrations of the prointlammatory cytokines such as IL6, IL-8 and neutrophil were measured at the time points of T1 (10 rain after the trachea eannula), T2 (5 min after bypass), T3 (time of the surgery termination), T4 (2 hours after the surgery) and T5(24 hours after the surgery). Results: After CPB, the concentrations of IL-6 at T3, T4 and IL-8 at T4 had increased significantly (P 〈 0.05 ), but the changes of the concentration of the control group were more significant ( P 〈 0.05 ), with a decrease close to the normal level at Ts, but IL-6 concentration was a little higher than the treated group; the neutrophile quantity increased after CPB, but the difference was not significant between the two groups ( P 〉 0. 05). Conclusions: Preoperative application of the ligustrazine hydrochloride in infants with congenital heart disease could reduce the peak level of IL-6 and IL-8 after CPB.
