摘要
目的探讨反复咳嗽患儿不同年龄组与居住地、过敏原及免疫功能的关系。方法选择符合诊断标准的277例患儿,按照年龄分A组(≥1~3岁)、B组(~7岁)和C组(~14岁),按居住地分为城镇及乡村组。进行T细胞亚群、免疫球蛋白、体外变态反应和总IgE检测。结果(1)A组是反复咳嗽的高发年龄段。(2)反复咳嗽居住地城镇组明显高于乡村组。(3)吸入性过敏原过筛试验(Phadiatop)及尘螨特异性试验(d1)阳性率高的年龄段为B、C两个组,食物过敏原过筛试验(Fx5E)阳性率高的年龄段为A组,血清总IgE(TIgE)阳性率高的年龄段为A、B两个组。(4)三组免疫球蛋白(IgA、IgG、IgM)IgG、IgM及A组IgA检测值正常者比率明显少于检测值降低者。结论(1)年龄愈小,反复咳嗽发生率愈高,幼儿易于食物过敏,大年龄组易于吸入物过敏。(2)空气质量差和接触感染性疾病增加反复咳嗽几率。(3)体外变态反应阳性者易患哮喘。(4)有反复呼吸道感染征象而免疫功能正常的过敏体质患儿,应注意排除存在呼吸系统变态反应性疾病的可能。
Objective To investigate the relationships between different ages, dwelling places, allergen and immune function in children recurrent cough. Methods According to the diagnostic criteria, 277 patients were divided into 3 groups of A(≥1-3y), B(-7y ) and C(-14y). A total of 156 cases lived in urban and 121 cases in city. T lymphocyte subsets, serum immunoglobulin were detected and the allergy tests were applied in vitro. Results (1) The patients in group A were more susceptible to the recurrent cough. (2) The incidence of patients lived in urban was significantly greater than that in city. (3) The positive rates of Phadiatop and dl in group B and C were significantly higher than those in group A, which of Fx5E were significantly higher in group A and which of total IgE(TigE) were obviously increased in group A and B. (4) The number of patients with IgG and IgM normal levels in 3 groups and that with IgA normal level in group A were significantly lower. Conclusion (1) The incidence of recurrent cough is decreased with the increase of age. Infants tendes to be allergic to foods while preschoolers and school age children are prone to be allergic to inhalant. (2) Air pollution and contact with contagious disease may increase the incidence of recurrent cough. (3)Recurrent cough children with positive allergy in vitro may be regarded as a high risk subjects suffering from asthma. (4) The respiratory tract allergy diseases can be excluded in the allergy patients with recurrent respiratory tract infection and normal immune function.
