摘要
目的探讨美容外科技术对接受颜面部皮肤扩张术的患者进行后期美容修复的效果。方法运用自体脂肪颗粒注射移植，矫正扩张区凹陷畸形；掀起皮瓣，将皮瓣上移，并与深部骨膜表面组织固定悬吊，纠正下睑外翻，口唇和鼻翼牵拉移位；采用美容缝合技术，矫正手术切口瘢痕增生、变宽、过于显露等；采用自体单株毛发移植，部分或全部再造眉、胡须；光子脱毛术纠正术区毛发异位。结果本组共355例患者，均随访2个月至3年。采用自体脂肪颗粒注射移植矫正扩张区凹陷畸形45例，全部病例均得到明显纠正；切口瘢痕修整256例，其中单纯瘢痕切除缝合216例，同时行转移皮瓣掀起、深部固定悬吊40例，除14例症状部分改善外，其余202例患者均获得明显改善。部分或全部再造眉、胡须28例，采用光子脱毛治疗毛发异位26例，均疗效满意。结论对接受颜面部扩张术治疗后的患者进行进一步美容修复有其必要性，可以取得良好效果。
Objective To explore further cosmetic reconstruction for the patients after facial expansion by modern cosmetic surgery technique in order to achieve more satisfactory results. Methods The depression deformities from the tissue expansion, including the facial sketch and asymmetry deformities, were repaired by the antogenetic lipochondria injection; The correction of the lower eyelid ectropion and the displace of angle of month and nasal ala caused by drag was performed by lifting and removing upward the flap and then it was fiaxed and suspened on the surface tissue of deep periosteum; Postoperative scar hyperplasia, widen and excessive expose of incisions were corrected by the normal cosmetic excision and suture technique ; The reconstruction of partial or full eyebrow or beard was caMed by the autogenetic hair transplantation ; The hair allotopia was repaired by photon depilating method. Results All 355 cases were followed up for 2 - 36 months postoperatively. The depressions deformities of 45 cases from the tissue expand repaired by the autogenetic lipochondria injection achieved satisfactory results. Of 256 cases with incision scar, 216 cases were repaired by simple scar cosmetic excision and suture, 40 cases were corrected by skin flap lifting and deep fixation and suspension, all improved obviously except 14 cases with partial improvement. All 28 cases with partial or full eyebrow or beam reconstruction by the autogenetic hair transplantation and 26 cases of hair allotopia with photon depilating method cases were satisfied with the excellent results. Conclusion It is necessary for the patients who were performed facial expansion to have further cosmetic reconstruction by modern cosmetic surgery technique.
