摘要
目的了解肿瘤内科患者中心静脉导管相关感染情况。方法回顾性调查1086例进行了导管培养的患者,分析导管相关性感染与原发肿瘤、导管留置静脉、导管留置时间、导管类型、病原学的关系。结果1086例患者中,导管培养阳性59例(5.43%),分离出病原菌27株,其中革兰氏阳性菌14株,革兰氏阴性菌8株,真菌5株;发生导管局部定植45例(4.14%),穿刺部位感染9例(0.83%),菌血症5例(0.46%)。肿瘤内科患者中心静脉导管感染与导管留置静脉、导管类型、导管留置时间、原发肿瘤类型有关。结论肿瘤内科患者中心静脉导管感染与导管细菌定植等因素有关。正确选择导管留置静脉和导管类型,加强置管操作与维护管理,尽可能缩短导管留置时间,加强患者营养,提高免疫力可降低导管感染率。
Objective To investigate the prevalence and nature of catheter-related infections in cancer patients with central venous catheter. Methods The relationship between catheter-related infections and types of original tumor,types of intubation veins,catheter indwelling time ,catheter types ,etiology were retrospectively analyzed based on the data of 1086 cases of catheter cultivation. Results In 1086 patients,59 patients were infected. The isolated microbe strains including 14 strains of Gram-positive bacteria,8 strains of Gram-negative bacteria and 5 of fungi. Pathogens propagated around the catheter in 45(4.14%) patients. On the puncture parts,9(0.83%) patients were infected and 5(0.46%) patients suffered from bacteremia. The infection rate was different among patients with different types of catheters,indwelling time and types of original tumor (P〈0.01). Conclusion The propagation of external bacteria is the major cause of catheter-related infections. It is suggested to select proper veins and types of catheter,strengthen the management of intubation and catheter maintenance,shorten catheter indwelling time as possible as we can,enhance the patients' nutritional status and immunity in order to reduce catheter-related infection rate.
