摘要
目的探讨2型糖尿病(T2DM)家系正常糖耐量一级亲属(一级亲属)血清脂联素和抵抗素表达水平的变化,研究脂联素和抵抗素与胰岛素抵抗(IR)的相关性。方法收集广西壮族地区T2DM家系45个,在排除糖尿病和糖耐量损害的前提下,选择一级亲属为一级亲属组(83例),先证者的配偶或其同胞的配偶为对照组(76例),检测所有受试者的空腹血糖(FPC)、空腹胰岛素(FINS)、胰岛素原(FPI)、血脂、脂联素和抵抗素水平。采用稳态模型法(HOMA)计算胰岛素抵抗指数(HOMA—IR)。结果一级亲属组的甘油三酯(TG)、FPI、FINS、HOMA-IR和抵抗素水平显著高于对照组(P〈0.05或P〈0.01),高密度脂蛋白(HDL)和脂联素显著低于对照组(P〈0.05)。多元线性逐步回归分析显示,FPG、体质指数(BMI)、脂联素、抵抗素是影响T2DM家系一级亲属胰岛素抵抗的独立危险因素。结论T2DM家系一级亲属在未发生糖尿病时已存在胰岛素抵抗,脂联素、抵抗素可影响T2DM家系一级亲属IR的程度。
Objective To investigate the changes of serum adiponectin and resistin levels in the first-degree relatives of type 2 diabetic families and the relationship of adiponectin, resistin with insulin resistance. Methods 45 type 2 diabetes families in Guangxi Zhuang were recruited. The observation group comprised with 83 first-degree relative who with normal glucose tolerance(NGT). The normal control group comprised with 76 proband' s spouse or spouse of proband' s siblings with NGT. The levels of fasting blood glucose, insulin, proinsulin,lipid, adiponectin and resistin were measured among two groups. Insulin resistance was calculated using the homeostasis assessment(HOMA). Results The levels of TG,fasting insulin (FINS), fasting proinsulin( FPI), resistin and HOMA-IR in first-degree relatives were significantly higher ( P 〈0. 05 or P 〈 0. 01 ) and the HDL, adiponeetin level in these population was significantly lower( P 〈 0.05 )than normal control. Multiple stepwise regression showed that fasting blood glucose (FPG), body mass index(BMI), adiponectin and resistin were independent risk factors affecting insulin resistance in first-degree relatives. Conclusion Insulin resistance had appeared before the development of diabetes in first-degree relatives of patients with type 2 diabetes. Adiponectin and resistin may affect the IR of first-degree relatives of patients with type 2 diabetes.
