支气管哮喘大鼠模型大小气道病理学的动态变化及预防性吸入糖皮质激素的干预作用 被引量:2

Effects of preventive inhaled corticosteroid on dynamic Morphological changes in the airways of bronchial asthma rats

摘要 目的探讨支气管哮喘(简称哮喘)大鼠气道重塑的形态学动态变化及吸入糖皮质激素的干预作用。方法按随机数字表法将72只清洁级雄性Wistar大鼠分为对照组、哮喘组(卵白蛋白)、哮喘干预组(布地奈德+卵白蛋白),每组大鼠24只,分别于雾化激发后第7天、第28天、第35天处死每组大鼠中8只进行气道形态学观察,并采用病理图像分析系统测量大鼠的气道形态学参数。气道被分为大气道和小气道两个等级进行分析。结果大气道病理学变化:与相应正常对照组相比,28d、35d哮喘组内管壁厚度、平滑肌厚度明显增厚(P<0.05或<0.01);治疗28d、35d后管壁厚度、平滑肌厚度与相应哮喘组相比明显减轻(P<0.05或<0.01),接近于正常对照组水平(P值均>0.05)。小气道病理学变化:与相应正常对照组相比,28d、35d哮喘组内管壁厚度、平滑肌厚度、胶原沉积明显增厚(P<0.05或<0.01);经治疗后28d、35d组平滑肌厚度与相应哮喘组相比差异无统计学意义;而胶原沉积在治疗28d、35d组与相应哮喘组相比明显减少(P<0.01);内管壁厚度治疗35d组与相应哮喘组相比差异开始下降,但仍高于正常对照组(P值均<0.05)。结论气道重塑累及整个气道,大气道发生重塑较小气道出现早且较重。早期预防性吸入糖皮质激素治疗可有效的干预哮喘大鼠气道壁胶原的沉积,但对气道平滑肌层的增厚干预作用较弱;吸入激素对大气道重塑的抑制作用较小气道作用明显。 Objective To explore the establishment of a bronchial asthma(asthma) model in rats and the effects of preventive inhaling cortieosteroid(ICS) on airway remodeling by observing the morphological changes of the airway in different stages. Methods Seventy-two Wistar rats were divided into control group, asthmatic group (sensitized by OVA) and steroids treated group(treated by steroid and sensitized by OVA), each group had 24 rats. The subjects were observed at 3 phases: 7 days, 28 days, 35 days after activation. General histological changes were observed using HE and Masson trichrome stained sections. The parameter of the internal perimeter (Pi), bronchial smooth muscle area (WArn), internal wall area (WAi) , and Weol collage deposition(Wcol) of the airway were measured by computer-assisted image analysis system. And the membrane airways were divided into 2 grades (large and small airways) to be analyzed. Results The , remoldling histological changes of large airway: the thickness of airway inner wall, smooth muscle and(WAi/ Pi,WAm/Pi) in asthmatic groups of 7 days, 28 days and 35 days were significantly higher than the corresponding control group( P〈0.05 or d0.01). After treated with budesonide (BUD), the Wcol/Pi of 28 days and 35 days BUD treated groups decreased obviously compared with the corresponding asthmatic group ( P 〈0.05 or 〈0.01). The WAi/Pi,WAm/Pi of 28 days and 35 days BUD treated groups became to thinner and had no significant diference with that of corresponding asthmatic group. The remoldling histological changes of small airway: the WAi/Pi,WAm/Pi and Wcol/Pi in asthmatic groups of 28 days and 35 days were significantly higher than the corresponding control group( P 〈0.05 or P 〈0.01). After treated with BUD the WAm/Pi of 28 days and 35 days BUD treated groups had no significant diference with that of corresponding asthmatic group( P〈0.05). But the Wcol/Pi of 28 days and 35 days BUD treated groups decreased
