早期内毒素血症大鼠电镜心肌损害及肌酸磷酸激酶同工酶/心肌肌钙蛋白-I的改变 被引量:3

Changes of MB Isoenzyme of Creatine Kinase/Cardial Troponin I and Myocardial Pathological Damage of Rat with Endotoxemia at Early Stage

摘要 目的探讨内毒素血症早期大鼠心肌损害和CK-MB/心肌肌钙蛋白-I(cTnI)水平的改变。方法将60只SD大鼠随机分为9 g/L盐水组(NS组,n=12)和脂多糖组(LPS组,n=48),二组又分为注射0.5、1.0、2.0、3.0、12.0、24.0 h 6个亚组。LPS组腹腔注射5 mg/kg LPS,NS组腹腔注射9 g/L盐水1 mL。在注射后不同时间点处死大鼠,采其腹主动脉血3 mL,检测其血清CK-MB和cTnI,同时取其心肌标本,行HE染色,制作电镜切片,通过光镜及电镜观察二组大鼠心肌结构改变。采用SPSS 13.0软件进行统计学分析。结果LPS注射15 min后,大鼠活动减少、反应慢;1 h后,活动差,反应差;3 h后,出现少动,毛发光泽度减弱;24 h出现进食减少,叫声减弱,对刺激反应弱,毛发光泽度差。与NS组比较,LPS注射后1.0、2.0、3.0、12.0、24.0 h CK-MB显著增高(Pa<0.05,0.01);LPS注射后3.0、12.0、24.0 h cTnI显著增高(Pa<0.05,0.01)。LPS注射后0.5 h电镜下见大鼠心肌出现病理改变,心肌细胞纤维变细,少部分肌丝断裂,排列紊乱;线粒体大量增生、肿胀,呈空泡样变,嵴断裂、排列紊乱;核膜完整,但见核周空晕,常染色质、异染色质淡染,1.0 h时光镜下可观察心肌纤维肿胀,胞质淡染,纤维少量断裂,随着时间的推移,心肌的病理改变更明显。结论内毒素血症早期大鼠0.5 h即出现心肌损害,血清CK-MB、cTnI升高稍晚,随时间增高逐渐明显。 Objective To explore the myocardial damage and the changes of MB isoenzyme of creatine kinase ( CK - MB) and cardial troponin Ⅰ (cTnⅠ) of rat with endotoxemia at early stage. Methods Sixty SD rats were randomly divided into lipopolysaccharide (LPS) group ( LPS group, n = 48 ) and control group( n = 12 ). Rats in LPS group were injected intraperitoneally with LPS ( 5 mg/kg) and then divided into 6 subgroups according sacrificed time after injection (0.5,1.0,2.0,3. 0, 12.0,24.0 h). CK - MB/cTnⅠ in blood was determined with chemiluminescent technique and myocardial pathological damage was observed under the light and transmission electron microscope. Control group was subjected to normal sodium and divided randomly into the same subgroups too. Results The rat diminished their action after beening injected endotoxin 15 minutes, and their reaction were slow. One hour later, they were short of action and reaction. Three hours later, they almost did not move at all, and the glossiness of their pelage were faint. Twenty - four hours later, their foodintake was diminished, their shout were weakened, and their response to stimulus were very thin, the glossiness of pelage were insufficient. Compared with control group, CK - MB increased significantly at 1.0,2.0,3.0,12.0,24.0 h ( Pa 〈 0.05,0.01 ) , and cTnl increased significantly at 3.0,12.0,24.0 h ( Pa 〈 0.05,0.01 ). The myocardial damage could be observed in 0.5 h LPS subgroup by electron microscopy, and the fibrilla of myocardial cell were thinningzed, a few of myofilament were broken, and arranged chaoticly ; cytochondriomes were hyperplasy and swelled, and there were vacuolus in them, their cristae were disrupted, arranged chaoticly; the nucleolemma was complete, but there were haloes aroud nucleus, the euchromatin and heterochromatin dyed lightly. The myocardial damage also could be observed at 1.0 h in experimental subgroup by light microscopy : the fibrilla of myocardial cell were swelled, endochylema dyed lightly, and
