摘要
目的评价晶状体囊袋张力环(capsular tension ring,CTR)在外伤性晶状体半脱位手术中的应用价值。方法对19例外伤性晶状体半脱位患者行白内障超声乳化术。有玻璃体疝者先用玻璃体切割器切除前房内及晶状体赤道部玻璃体。连续环形撕前囊,植囊袋张力环于晶状体囊袋内,然后行超声乳化摘除白内障,囊袋内植入后房型人工晶状体。合并青光眼者同时行青光眼白内障联合手术。结果术后19例患者人工晶状体均为正位。随访3～6个月。矫正视力>0.8者6只眼,0.4～0.8者12只眼,0.3者1只眼,全部患者术后眼压均在正常范围。结论囊袋张力环在晶状体半脱位白内障手术中的应用,使手术过程更为安全,它阻挡玻璃体前移,避免人工晶状体偏位。并可防止晶状体悬韧带断离范围扩大,减少手术并发症,并且有助于视力快速恢复。
Objective To evaluate the application of capsular tension ring（CTR）in the traumatic cataract sugery for zonular dialysis. Methods Nineteen cases of partially traumatized zonular dialysis patients were managed with CTR after continuous curvilinear capsulorrhexis,followed with phacoemulsification and posterior chamber intraocular lens（IOL）implamtation, combined with vitrectomy when vitreous hernia in anterior chamber or trabeculectomy for glaucoma with cataract. Results The IOLs in nineteen cases were basically in the normal position without tilting or obvious decentration.3- 6months after surgery, corrected visual acuity was〉0.8in 6 eyes,0.4-0.8in12eyes and 0.3in 1 eye. Postoperative intraocular presure in all the eyes were normal. Conclusion Capsular tension ring implantation is safe and effective in the patient of lens subluxation during phacoemulsifiction.It can prevent the IOL decentration and zonular dialysis enlarge, stebilize the vitreous base,decrease the vitreoretinal complication and allow a quicker visual recovery.
