2型糖尿病患者下肢动脉硬化与早期肾损害的相关性研究

摘要 目的分析2型糖尿病(T2DM)下肢动脉硬化与早期肾损害的相关性。方法选择尿常规检测尿蛋白阴性的T2DM患者429例,平均年龄(62.3±12.4)岁。应用ES-1000SPM多普勒血流探测仪测定踝肱指数(ABI),并测定微量白蛋白尿(MAU)。结果以MAU为因变量,ABI、体质量指数(BMI)等为自变量进行多元线性回归分析显示,MAU与ABI负相关(标准化回归系数-0.118,P=0.02),与HbAlc、BMI、Cr正相关(标准化回归系数分别为0.191、0.122、0.326,均P<0.05)。结论MAU对下肢血管病变具有预测作用,二者密切相关,应加强对T2DM患者MAU及ABI的筛查,并给予积极的早期干预治疗。 Objective To study the relation of lower extremity vascular disease to diabetic kidney disease in type 2 diabetic patients. Methods A total of 429 patients with type 2 diabetes mellitus with an average age of( 62.3 ± 12.4 ) years were enrolled. The ankle brachial index (ABI) in all cases was measured by ES -1000SPM Doppler blood flow detector, and microalbuminuria was as- sayed. Results Multiple regression analysis showed that the glycosylated hemoglobin, baric index,and creatinine (Cr) were positively correlated with microalbuminuria (standardized regression coefficient 0. 191, 0. 122, 0. 326, respectively, all P 〈0. 05 ). The ABI was negatively correlated with microalbuminuria ( standardized regression coefficient - 0. 1188, P 〈 0. 05 ). Conclusions Microalbuminuria plays a predicting role in prognosis of lower extremity vascular disease; they are closely correlated. Should be screening and active early treatment of microalbuminuria and ABI in patients with T2DM should be strengthened.
