Repair of digital end defects by reverse proper digital artery island flaps

摘要 目的探讨指固有动脉逆行岛状皮瓣修复指端缺损的效果。方法自2004年10月至2007年3月,应用指固有动脉逆行岛状皮瓣修复12例指端皮肤缺损患者。设计以一侧远端指固有动脉作为血管蒂,供区在近节手指的侧方,不带指固有神经的皮瓣。将该皮瓣向手指远端转移修复指端的缺损,将携带的指固有神经背侧支与对侧的指固有神经近端行端端吻合。结果12例患者中,除1例患者因静脉血管危象而导致皮瓣坏死外,余11例患者其皮瓣全部成活,其中1例患者出现伤口感染,经换药后愈合。皮瓣成活者经3~12个月的随访,外形满意,术后9个月两点辨别觉平均为8.5mm。结论指固有动脉逆行岛状皮瓣是修复指端缺损的可行方法。 Objective To investigate the clinical results of repairing the defects of digital end by reverse proper digital artery island flaps. Methods From October 2004 to March 2007, 12 patients suffered from the defects of digital end were restored by reverse proper digital artery island flaps. The flap with the pediele of distal end of the lateral proper artery and without digital proper nerve was designed. Donor site was at the side on the proximal part of the same finger, and the flap was transferred to repair the defect of finger tip, and then end to end anastomosis was performed between the dorsal branch of digital proper nerve and proximal end of contralateral proper digital nerve. Results All 12 cases were survival except 1 case with necrosis due to venous embolism. Among the survival cases, there was 1 case that with wound infection which healed by changing dressing. After 3 - 12 months follow-up, all 11 patients were satisfied with the appearance and the two point discrimination was 8.5 mm at 9 months postoperatively. Conclusion It is an effective method to repair the defects of digital end by the reverse proper digital artery island flaps
