胃供血动脉多层螺旋CT成像的临床应用研究 被引量:2

摘要 目的:探讨多层螺旋CT血管成像(MSCTA)不同成像方法显示胃供血动脉的价值。方法:80例无腹部手术、无胃部疾病患者行16层CT进行腹部增强扫描和动脉期血管成像。统计容积再现(VR)、最大密度投影(MIP)及薄层最大密度投影(TSMIP)对胃供血动脉的显示率及显示状况。用配对χ2检验观察显示率的差异,用配对秩和检验观察显示状况的差异。结果:3种重组技术对胃左动脉及胃网膜右动脉的显示率均为100%。VR、MIP、TSMIP对胃右动脉的显示率分别为50.00%、70.00%、90.00%;对胃网膜左动脉的显示率分别为30.00%、60.00%、77.50%;对胃后动脉的显示率分别为6.25%、16.25%25.00%;对胃短动脉的显示率分别为10.00%、25.00%、40.00%。VR与TSMIP对胃右动脉、胃网膜左动脉、胃后动脉、胃短动脉的显示率的差异均有统计学意义(χ2值分别为30.47、28.92、10.67、19.20,P值均<0.05);VR与MIP对胃右动脉、胃网膜左动脉、胃后动脉、胃短动脉的显示率的差异均有统计学意义(χ2值分别为6.67、14.54、4.01、6.23,P值均<0.05);TSMIP与MIP对胃右动脉、胃短动脉的显示率差异有统计学意义(χ2值分别为10.00、4.10,P值均<0.05)。VR与MIP、VR与TSMIP、MIP与TSMIP对胃供血动脉显示状况的差异均有统计学意义(Z值分别为4.62、8.31、3.76,P值均<0.05)。结论:TSMIP显示胃供血动脉优于MIP及VR,VR显示胃供血动脉空间立体感强,TSMIP与VR结合可以较好的显示胃供血动脉的解剖结构。 Objective:To explore the clinical value of MSCTA in the visualization of gastric arteries. Methods:Dual-phase CTA using a Siemens Sensation 16 scanner was performed in 80 cases without abdominal surgery and stomach disease. CTA of gastric arteries were made using VR,MIP and TSMIP techniques. The frequency and degree of visualization of the feeding arteries of stomach were recorded and evaluated by two radiologists together. Results:The left gastric: artery and right gastroepiploic artery were all seen in each technique. In VR, MIP, TSMIP, visualization rates of right gastric artery were 50.00%, 70.00%, 90.00%, respectively ;left gastroepiploie artery, 30.00%, 60.00%, 77.50% ; posterior gastric artery, 6.25%, 16.25%, 25.00% ; short gastric artery 10.00%, 25.00%, 40.00%. There were significant difference in visualization rates of right gastric artery,left gastroepiploie artery, posterior gastric artery, short gastric artery between VR and TSMIP, as well as between VR and MIP. There were significant difference in visualization rates of right gastric artery, short gastric artery between MIP and TSMIP. There were significant difference in the degree of visualization of the feeding arteries of stomach among TSMIP, MIP and VR. Conclusion:The gastric arteries are displayed better on TSMIP than on VR and MIP. The gastric arteries have three-dimensional sense of space on VR .The anatomic features and the variations of gastric arteries can he accurately displayed by VR and TSMIP.
