富血小板血浆复合生物活性玻璃治疗牙周骨下袋的临床研究

摘要 目的比较生物活性玻璃(Bioactive Glass,BAG),BAG和引导组织再生术(GTR)联合应用,GTR、BAG、和富血小板血浆(PRP)联合应用三种手术方法治疗牙周骨下袋的疗效。方法21例患者的21颗骨下袋缺损牙随机应用BAG、BAG/GTR、BAG/GTR/PRP治疗,在术后6个月、12个月、18个月观察术牙在探诊深度(PD)、临床附着丧失(CAL)、临床附着获得、骨新生量的变化。结果三组患牙术后6个月、12个月、18个月的各项指标均比基线时明显改善(P<0.01),而在各观察时间三组间的各观察指标比较无统计学差异(P>0.05)。结论BAG/GTR/PRP、BAG/GTR、BAG均是治疗骨下袋的有效方法,应用PRP未发现有更好的临床效果。 Objective: To evaluate Bioactive glass, a combination of guided tissue regeneration (GTR) and Bioglass, a combination of platelet-rich plasma(PRP) , Bioglass and GTR which can improve the healing of periodontal intrabony defects. Methods :A total of 21 periodontal introbony defects were divided into 3 groups and were surgically treated with Bioglass, GTR/Bioglass, PRP/GTR/Bioglass. Changes in probing depth(PD) , CAL, clinical attachment gain and defect fill were evaluated at baseline, 6 months, 12 months and 18 months prospectively. Results: The results showed that at 6 months 3 groups presented with significant greater PD reduction, clinical attachment gain and defect fill risment than baseline(p 〈0.01 ). Interestingly, no significant difference was found between Bioglass alone GTR/Bioglass and GTR/PRP/ Bioglass(P 〉0.05). Conclusion:This study demonstrates that Bioglass , GTR/Bioglass, GTR/PRP/Bioglass can improve the healing of periodontal introbony defects. The results also show that using PRP has no additional benefit.
