椎弓根钉加单枚Cage融合器治疗腰椎滑脱症 被引量:1

Treatment of lumbar spondylolisthesis by using posterior pedicle screw instrumented reduction and single fusion Cage after decompressive laminectomy

摘要 目的探讨椎管减压椎弓根钉加单枚Cage融合器治疗腰椎滑脱症的疗效。方法采用椎板切除减压、椎弓根钉固定加单枚Cage植入融合治疗腰椎滑脱患者35例,观察术前、术后和随访期间Taillard指数、椎间隙高度、滑脱角、腰椎生理前凸角指数的变化。结果35例均获随访,时间18~60(36±4.8)个月。术后1周Taillard指数、椎间隙高度、滑脱角、腰椎生理前凸角与术前比较差异有显著性(P<0.05);术后1周与随访1年时各指标比较差异无显著性(P>0.05)。参照尹飚等疗效评价标准,优良率为91.4%(32/35),融合率为100%。无神经根损伤,无内固定材料松动和断裂,Cage融合器位置形态无改变,无脱出、下沉。结论椎管减压、椎弓根钉加单枚Cage融合器治疗腰椎滑脱,复位满意,效果良好;与2枚Cage相比,能获得更好的近期疗效。 Objective To analyse the treatment of lumbar spondylolisthesis by using posterior pedicle screw instrumented reduction and single fusion Cage after decompressive laminectomy.Methods 35 cases with lumbar spondylolisthesis who were operated by using posterior pedicle screw instrumented reduction and single fusion Cage after decompressive laminectomy were reviewed. Tailard index,Boxall index,the spondylisthesis angle,anterior process angle,and height of intervetebral space were studied.Results 35 cases were followed up for 1860 months (averaging 36±4.8 months).Postoperative Tailard index,Boxall index,the spondylisthesis angle,anterior process angle,height of intervetebral space were significant different to preoperation(P〈0.05),and the difference was not sigmificant between 1 week postoperatively and 1 year postoperatively(P〉0.05).According to Yinbiao′s Standard,the excellent and good rate was 91.4%,and the fusion rate was 100%. There was no nerve root injury and internal fixation loosening or breakage.Conclusions The treatment of lumbar spondylolisthesis by using posterior pedicle screw instrumented reduction and single fusion Cage after decompressive laminectomy provides excellent effect and good reduction. The short-term effect is better than two fusion cages.
