摘要
目的:探讨慢性胰腺炎(CP)合并胰腺癌(PC)的MRI特征,及其与单纯性CP的鉴别诊断。方法:回顾性分析经病理证实CP合并PC者16例的MRI资料,同期经病理或临床证实的单纯性CP者19例及腹部扫描未见异常15例患者作为对照。比较三组MR影像表现。结果:胰腺实质萎缩、主胰管扩张出现率及扩张程度、胰管串珠状改变、胰管截断征在CP合并PC组出现率最高,高于单纯CP组和正常对照组(P<0.05);胰管双段征在CP合并PC组高于单纯性CP组(P=0.035)。胰腺信号变化、分支胰管显示、胰管结石或腺体钙化、假性囊肿形成在CP合并PC组和单纯CP组间出现率无统计学意义(P>0.31),但胰腺信号改变、分支胰管显示,前两组均高于正常对照组(P<0.05)。结论:慢性胰腺炎合并胰腺癌有较特征性MRI表现,有助于与单纯性慢性胰腺炎鉴别。
Purpose: To study MRI features of chronic pancreatitis(CP) coexisting with pancreatic carcinoma (PC) and its differential diagnosis from CP noncoexisting with PC. Methods:There were 16 cases with CP and coexisting with PC confirmed by histopathology in this study. All of them had performed abdominal MR imaging. At the same duration, 19 cases with CP and non - coexisting with PC confirmed by pathology or clinic, and 15 normal subjects were served as control. The MRI findings in three groups were compared. Results:On MR imaging, pancreas atrophy, dilate of main pancreatic duct, bead - like change, cut off sign of main pancreatic duct and two segments sign were seen more common in patients with CP coexisting with PC than in patients with CP and non - coexisting with PC, and in control group (P 〈 0.05) . No statistical significance was found between patients with CP coexisting with and without PC in terms of the signal intensity patterns, presentation of branches of pancreatic duct, calculus or calcification within pancreatic duct and pseudocyst, respectively (P 〉 0.31). However, the signal intensity change of the pancreatic parenchyma and presentation of branches of pancreatic duct were significantly more common in patients with CP coexisting with and without CP than in control group(P 〈 0.05) . Conclusion: There were some MRI features for CP coexisting with PC. These features were helpful for differentiating it from CP non - coexisting with PC and for the detection and diagnosis of PC.
