脂微球前列地尔对单肺通气食管癌患者肺内分流及动脉氧合的影响 被引量:1

摘要 目的探讨脂微球前列地尔(lipo-PGE1)对单肺通气(OLV)下食管癌手术患者肺内分流率(Qs/Qt)及动脉氧合的影响。方法择期单肺通气全麻下行左侧开胸食管癌切除+胃食管吻合术患者40例,随机分为lipo-PGE1组(P组)和对照组(C组)各20例。麻醉诱导后P组恒速静脉泵注lipo-PGE12ng/(kg.min),C组以同样的容积速度泵入生理盐水。在侧卧位双肺通气时(T1)、OLV后15min(T2)、OLV后30min(T3)及术后第1天(T4)采集动静脉血样行血气分析并记录血流动力学指标。结果MAP,HR,CVP,FiO2,pH值变化趋势组间差异无显著性(P>0.05),各时点组间对比差异无显著性(P>0.05),Hb、Qs/Qt组间变化趋势及各时点对比差异无显著性(P>0.05),P组PaO2及氧合指数(PaO2/FiO2)在T3、T4较C组高(P<0.05)。术后并发症及ICU住院时间组间差异无显著性(P>0.05)。结论适当剂量静脉泵注Lipo-PGE1对单肺通气下食管癌手术患者肺内分流率无明显影响,不影响术中血流动力学,能改善OLV期间及术后动脉氧合。
