女性盆腔CT增强扫描的方法研究 被引量:3

摘要 目的:研究提高妇科盆腔CT增强效果的方法。方法:搜集我院临床诊断盆腔占位病变的女性患者资料75例,随机分为三组,行盆腔增强CT螺旋扫描。甲组和乙组均采用一次性团注法;丙组采用2次间隔团注法。使用Philips明几净Brilliance6螺旋CT,Medrad高压注射器,依病人公斤体重计算,按2ml/kg给予碘比乐370对比剂,最大剂量不超过120ml;扫描参数三组均采用5mm层厚和层间隔,6×1.5mm准直器,P=0.9。最后三组原始数据全部选用2mm层厚、1mm层间隔行离线重叠重建。由两位医师分别在金盘GDPACS诊断工作站盲法分析75例观测对象的CT图像资料,对增强效果进行分级评分;最后对所得三组对应数据进行组间t检验。结果:丙组评分最高且评优图像最多。三组数据兴趣点CT值平均值统计结果,丙组CT值最高;乙组相对于甲组仅髂内动脉的CT值变化有统计学意义;丙组相对于甲组髂内动脉、髂总静脉和子宫肌壁的CT值变化均有统计学意义;丙组相对于乙组,除子宫肌壁外,双侧髂内动脉和髂总静脉的CT值变化均有统计学意义。结论:采用2次间隔静脉快速团注法行妇科盆腔增强CT扫描,能大大提高盆腔动、静脉的强化水平,提高病灶与正常组织和血管之间的对比显示情况,更有利于病灶的检出和显示。 Objective:To study the method of improving the female intrapelvic CT enhanced scanning effect. Methods: 75 cases of female patients, being diagnosed as having pelvic occupying lesion, were divided into three groups at random and underwent pelvic helical CT enhanced scanning. The group A and B adopted once intravenous bolus injection, the third group adopted twice spaced intravenous fast bolus injection. We used Philips Brilliance 6 spiral CT, Medrad high injector and Iopamidol as contrast agent. For all three groups the same scanning parameters were used as 5mm sli pressure ce thick- ness and slice increment,6 〉( 1.5mm collimator,0.9 pitch. Then three group's raw data were off-line overlap reconstructed on the workstation using the parameters as 2mm slice thickness and lmm slice increment,and then 75 cases CT enhanced images were clinically graded and scored by two doctors on GD PACS diagnosis workstation by blind analysis. Finally t-test was used to compare the data of the three groups. Results:The third group had highest score and had most perfect images. For the average ROI CT values of the three group,the value was highest in the third group. There was statistical signifi- cance only between bilateral internal iliac artery CT values for the second group in comparison with the first group; there was statistical significance between bilateral internal iliac artery and common iliac vein,as well as uterus muscle CT values for the third group in comparison with the first group;excepting uterus muscle,there was statistical significance between bi- lateral internal iliac artery and common iliac vein CT values for the third group in comparison with the second group. Con- elusion:Female intrapelvic CT enhanced scanning when adopting twice spaced intravenous fast bolus injection can remark- ably improve the enhanced level of pelvic artery and vein,increase the distinction between lesions and normal tissues,as well as vessels,therefore this technique is beneficial to the detection and display of lesions.
