摘要
目的:探讨冠心病中医证候与冠状动脉造影结果的关系。方法:将229例冠心病患者进行中医分型和冠状动脉造影检查,对比分析中医证候与冠状动脉Gensini积分、病变支数的关系。结果:①证素研究中血瘀证出现的频率明显高于其他证候,且随着病变支数的增加有增高的趋势;分类证候研究中虚证以3支以上为主(66.7%),明显高于实证组(30.2%)与虚实夹杂组(47.1%);实证组单支病变为主(41.3%),高于虚证组(25.0%)和虚实夹杂证组(23.6%),冠状动脉病变累及支数呈现:虚证组>虚实夹杂证组>实证组;3者之间比较有统计学差异(P<0.05)。血瘀证与非血瘀证的研究显示,血瘀证在冠状动脉三支病变较非血瘀证出现的多,但二者无统计学差异。②Gensini积分研究显示:虚证积分高于虚实夹杂证和实证,即:虚证>虚实夹杂证>实证,3者之间有统计学差异(P<0.05)。血瘀证的积分高于非血瘀证,但二者未见统计学差异。结论:虚证和血瘀证的患者冠脉病变累及支数多,病变程度重,提示冠心病患者中医证候与冠状动脉病变支数、Gensini积分间可能存在相关性。
Objective: To discuss the relationship between TCM syndrome and the results of coronary angiography. Methods: After TCM syndrome differentiation and coronary angiography examination of 299 patients with coronary heart disease, the relationship among TCM syndromes, Gensini integral and lesion vessels of coronary was analyzed. Results: (1)The syndrome element study indicated that occurrence frequency of blood stasis was obviously higher than other syndromes, showed an increasing trend with lesion vessels. The syndrome classification research showed that those deficiency syndrome group, which had mainly more than triple lesion vessels (66.7%), were evidently higher than excess syndrome group(30.2%)and insufficiency and excess syndrome group(47.1%). Excess syndrome group, with mainly single lesion vessel (41.3%), was higher than insufficiency group(25%) as well as insufficiency and excess syndromes group(23.6%). The involved lesion vessels of coronary presented that insufficiency group〉 insufficiency and excess syndrome group〉 excess syndrome group, there was statistical difference among them(P〈0.05), blood stasis and non-blood stasis study revealed that in cases of triple lesion vessels, blood stasis had a higher occurrence frequency than non- blood stasis, but there was no statistical difference between them.(2)Gensini integral research showed that the integration of deficiency syndrome was higher than insufficiency and excess syndrome and excess syndrome, the order was insufficiency syndrome〉 insufficiency and excess syndrome〉 excess syndrome, statistical difference was found among them(P〈0.05). Integral of blood stasis was higher than non-blood stasis, but no statistical difference was found. Conclusion: Patients with deficiency syndrome and blood stasis have more involved lesion vessels of coronary and higher lesion degree, suggesting that there may be correlation amog TCM syndrome, lesion vessels and Gensini integral of coronary.
