深低温冻存对大月龄胎儿带瓣血管组织免疫原性的影响

The effect of cryopreservation on the immunogenicity of homograft valved conduits from late-pregnancy fetuses

摘要 目的通过研究深低温保存前后大月龄胎儿主、肺动脉带瓣血管组织相容性白细胞抗原(HLA)的表达情况,分析深低温保存对组织HLA表达的影响。方法取10例脑死亡胎儿主、肺动脉带瓣血管,胎龄28~38周;每例随机分为两部分。一部分按冻存常规方法降温,-196℃长期保存者作为实验组,另一部分为对照组,未作冻存。利用免疫组化方法观察冷冻前后HLA-Ⅰ、Ⅱ类抗原和内皮细胞的变化情况。结果新鲜血管壁和瓣膜内皮表面均表达Ⅰ类抗原,而Ⅱ类抗原在血管壁内皮下层和瓣膜基质中表达。深低温冻存后Ⅰ类抗原在血管壁和瓣膜内皮表面消失,Ⅱ类抗原在相应组织中表达减弱。Ⅱ类抗原中内皮细胞抗原存在于冻存前后的血管表面,而在瓣膜表面冷冻后消失。结论深低温冻存后大月龄胎儿同种带瓣血管免疫原性降低。 Objective To analyze the histocompatibility leukocyte antigen(HLA) expression on aortic and pulmonary homografts valved conduits (HVCs) from late-pregnancy fetuses and evaluate the effect of cryoprcservation on immunogenicity of cadiac homografts. Methods Aortic and pulmonary HVCs were procured from 10 brain death fetuses aged 28 -38 (31.8 ± 3.46)weeks. Every specimen was randomly cut into two portions. One portion was preserved by routine hypothermal method to reduce the temperature and kept at - 196T; over a long period of time as the experimental group, and another portion as the control group. HLA Class Ⅰ , Ⅱ expression and endothelial cell variance were observed with immunohistochemistry before and after cryopreservation. Results HLA class Ⅰ could be demonstrated in the fresh conduit walls and on the valves surface, and HLA class Ⅱ could be demonstrated in the sub-endothelial layer and in the value matrix. After cryopreservation and thawing, HLA class Ⅰ could not be demonstrated in conduits walls or on the valve. HLA class Ⅱ expressions were attenuated in the corresponding tissue. Endothelial cells antigen expressions were well demonstrated in vessel walls even after cryopreservation, but could only be seen in fresh valves. Conclusions The immunogenicity of homograft aortic and pulmonary conduits and valves from late-pregnancy fetuses is weaker after cryopreserration.
