摘要
目的探讨瞬时波强度(WI)评价糖尿病和糖尿病合并高血压患者颈动脉血管内皮功能及心功能改变的应用价值。方法对单纯糖尿病组(DM)86例、糖尿病合并高血压组(DM+EH)87例、正常组对照组91例,用WI测量系统实时跟踪描记管壁运动轨迹,对所得参数值行统计学分析。结果血糖值(FSB)、脉压值(PP)、前向压缩波峰值W1、前向膨胀波峰值W2均为:DM+EH组最高,明显高于正常组和DM组(P<0.01),DM组低于正常对照组(P<0.05);内-中膜厚度(IMT)、僵硬度β、压力应变系数Eρ、脉搏波速度PWVβ在正常对照组、DM组及DM+EH组之间呈增高趋势,顺应性AC呈降低趋势(P均<0.05)。W1、W2与IMT呈正相关(r=0.226,0.214,P<0.01)。结论WI可作为综合评价DM和DM+EH患者动脉功能的新参数。
Objective To assess value of wave intensity (WI) in evaluating vascular function of carotid artery and the corresponding changes in heart function in patients with diabetes mellitus or combined with hypertension. Methods A total of 86 patients were in diabetes mellitus group (DM), 87 were enrolled in diabetes mellitus with hypertension group (DM+ EH), and 91 healthy volunteers were taken as control group. WI was performed to follow the movement tracks of the artery wall, and the parameters were measured. Results Fasting serum glucose (FSB) and pulse pressure (PP) were obviously higher in EM+EH group than in DM group (P〈0. 01) and control group. Prorsad compressed wave peak W1 in DM+EH group was higher than in DM+EH group and control group. Prorsad expanded wave peak W2 in DM+EH group increased more obviously than in the control group and DM group. Internal-media thickness (IMT), stiffness β, pressure strain coefficient Eρ and pulse wave velocity (PWV) β increased gradually in the control group, DM group and DM+EH group, while AC reduced correspondingly (both P〈0. 05). W1 and W2 were positively related to the IMT. Conclusion W1 can be used as a new method to evaluate vascular function of carotid artery in patients with DM and DM combined with EH.
