摘要
目的:了解新农合实施5年来,农民对新农合认知的动态变化情况,为正确评价新农合的实施效果及其实施方案的进一步完善提供科学依据。方法:采用多阶段分层整群随机抽样法抽取婺源县3个乡镇9个行政村,每村各取70户进行入户调查,将5次调查结果进行纵向对比研究。结果:本次调查有100.00%的农户知道新农合;知道国家为每位参加新农合的农民出资的仅占13.25%,而知道国家出多少钱的仅占3.64%,均为5次调查最低值;认为新农合住院费用的报销过程方便的占94.79%,认为住院费用报销时工作人员态度较好以上的占81.04%,总体均呈上升趋势;认为药价升高的占24.77%,总体呈先升后降的趋势;74.83%的农民认为新农合是公平、公正、公开的,总体呈上升趋势;农民对新农合带来的好处的认识中,前两位仍是"生大病有保障"和"医药费可报销一些",分别占80.96%和34.44%;农民对新农合的担心中,83.28%的农民回答不知道,高于前4次调查结果,而"报销比例低"的占11.09%,依然排在第2位。本次调查愿意继续参加新农合的农民占96.19%,参加意愿总体呈上升趋势。结论:婺源县在实施新农合5年后,农民都知道了新农合,对新农合报销及定点医疗机构服务的满意度较高,对新农合的担心减少。但对新农合的政策及具体方案不了解的农民比例呈上升趋势,需引起高度重视,并加大对农民宣传教育工作的力度。
Objective To know the dynamic changes in farmers＇ cognition of the new rural Cooperative Medical System （CMS） after five years＇ implementation of it and to offer scientific basis for correctly evaluating the implementation effect of the new rural CMS and further improving the scheme of the new rural CMS. Methods An investigation was carried out by interviewing farmers in their homes. The multi-phase stratified cluster random sampling has been adopted to select 9 administrative villages from 3 townships with 70 families from each village. The five surveys＇ results are used for a longitudinal comparative study. Results 100. 00% of the farmers know the new rural CMS in this survey. Only 13.25%of them know that the government provides funds and only 3. 64% of them know how much money the government provided, which are both the lowest proportion of all the five surveys. 94. 79% of them think it convenient to submit the hospitalization expenses,81.04% of them satisfied with the staff＇s service,both of them showing ascending trends. 24.77% of them think that drug prices have increased,which shows a descending trend after an ascending trend. 74.83%of the farmers think that the new rural CMS is fair,just,and open,which shows an ascending trend. Among the new rural CMS＇s advantages to the farmers,the top two still are ＂it ensures certainly to treat serious disease＂ and ＂they can wine out some medical fee＇,resoectivelv accounting, for 80. 96% and 34.44%. In worries and suggestions to the new -type rural CMS,83.28o/oo of them selected ＂having no idea＂,whieh is higher than the results of the previous four surveys,while 11, 09% of them selected ＂the proportion of wiping out is low＂,which still ranked No. 2. 96.19% of them expressed willingness to continue participating in next- year CMS which shows an upward trend.
