摘要
目的比较肱骨干骨折采用顺行和逆行交锁髓内钉的治疗效果。方法回顾性分析2005年1月至2008年10月采用顺、逆行交锁髓内钉治疗肱骨干中段骨折53例。结果全部病例获得6～28个月的随访，平均12．4个月。采用顺行入路（A组）28例，平均愈合时间为14．9周；肩关节功能按Neer评分，优良率89．3％；逆行入路（B组）25例，骨折均Ⅰ期愈合，平均愈合时间为13．4周，肩关节功能优良率100％。两组肘关节功能均正常。结论逆行交锁髓内钉治疗肱骨干中段骨折在骨折愈合及对肩关节功能影响方面优于顺行髓内钉固定。
Objective To compare the clinical effects of humeral shaft fractures with closed reduction and the use of antegrade and retrograde locked intramedullary nailing in the treatment. Methods From January 2005 to October 2008, fifty - three patients with the humeral shaft fracture were divided into two groups randomly. 28 and 25 patients were respectively treated with anterograde and retrograde interlocking intramedullary nails. Results All patients were followed up between 6 months and 28 months. The fracture union occurred in fifty - two patients. The fracture union was delayed in one patient. The average healing time was 14.9 weeks in A group, 13.4 weeks in B group. According to Neer shoulder functional score system ,the good rate is 89.3% in A group ,and 100% in B group. The excellent result was achieved of elbow function in both groups. Conclusion Closed reduction and retrograde interlocking intramedullary nail treatment of humeral shaft iractures in the fracture healing and functional rehabilitation of the shoulder joint is superior to the treatment with antegrade intramedullary nail.
