摘要
目的探讨改良腓骨(皮)瓣在一期修复胫骨感染性骨及皮肤缺损中的应用及效果。方法对9例外伤致胫骨感染性骨及皮肤缺损采用经小腿内侧肌间隙通道的改良腓骨皮瓣交腿移植方法.一期修复小腿皮肤与胫骨骨缺损。皮肤缺损面积最大20cm×15cm,胫骨缺损最长18cm。结果本组9例中,手术均获成功,伤口一期愈合8例,二期愈合1例;伤口愈合时间为10~21d,平均18d。随访时间为0.5~4.0年,平均2.5年。双下肢均可负重行走,步态基本正常,膝、踝关节屈伸活动正常.恢复正常生活与工作。结论经小腿内侧肌间隙通道的改良腓骨皮瓣交叉移植一期修复胫骨感染性骨及皮肤缺损,方法安全可靠、成功率高,体位舒适,是保肢的有效方法。
Objective Fibula (skin) flap in the repair of an infected tibial bone defects in the skin and the effect of the application. Methods Nine cases of traumatic bone infection caused by percutaneous tibial defect medial calf muscle by using the modified gap-channel cross-leg flap fibula transplantation method to repair a calf skin and bone defect of tibia. The largest area of skin defect of 20 cm × 15 cm, the longest tibial defect 18 cm. Results In 9 cases, surgery had been successful, a healing wound 8 cases, 21 cases of healing; wound healing time was 10 - 21 d, an average of 18 d. Follow-up period of 0.5 to 4.0 years, an average of 2.5 years. Weight-bearing walking can be both lower extremities, gait normal, knee, ankle flexion and extension activities in normal and resume normal life and work. Conclusion The medial calf muscle gap improved channel cross fibula flap graft to repair a tibial bone infection skin defects, safe and reliable method, the success rate is high, a comfortable position, is an effective method of limb salvage.
