摘要
目的探讨贲门癌外科治疗的最佳手术径路。方法对我院2007年9月-2009年5月57例经腹-左膈肌切口行贲门癌根治术进行总结分析。结果本组57例中,切缘无癌残留,切除淋巴结411枚,转移率为54.99%(226/411)。其中,腹腔淋巴结转移率为70.18%(40/57),胸腔淋巴结转移率为24.56%(14/57)。未发生肺部并发症。腹部切口感染2例,左侧胸腔积液一例(1.75%),无围手术期死亡。结论经腹-左膈肌切口行贲门癌根治术是一种较理想的手术径路。
Objective To discuss the best surgical approach of in patients with cardiac carcinoma. Methods From September 2007 to May 2004, 57 patients with cardiac carcinoma were retrospectively analyzed on the abdomen-laevdiaphragmafic muscle incision. Resuits Among the 57 patients, without thecarcinoma left in cutting edge, 411 absorbent glands were resected. The overall mestasis rate was 54.99% （226/411 ）. Of the total, absorbent glands of mestasis rate was 70. 18% （40/57） in abdominal cavity and was 24. 56% （14/57） in thoracic cavity. Complication of the lung didn＇t happen, the infection of abdominal incision happened to 2 cases, left pleural effussion happened to 1 case （ 1.75% ）, there was no patientsdead in the ambioperation-stage. Conclusion The abdomen-laevdiaphragmatic muscleradical correction for cardiac carcinoma is the proper surgical approach.
