髓过氧化物酶指数在急性白血病鉴别诊断中的应用 被引量:4

摘要 目的探讨髓过氧化物酶指数(MPXI)与血涂片过氧化物酶(POX)染色结果的相关性,及MPXI指数在白血病早期诊断中的价值。方法以健康体检人群为正常对照组,应用ADVIA2120全自动血细胞分析仪分别检测正常人(126例)、急性非淋巴细胞白血病(100倒)、急性淋巴细胞白血病(46例)及严重感染患者(68例)的MPXI值,同时对其中20例初发急性白血病人的血涂片进行POX染色,检测POX阳性细胞百分率和积分。结果急性非淋巴细胞白血病组的MPXI值(10.27±12.80)较正常对照组(-0.48±2.49)显著升高(P〈0.01);急性淋巴细胞白血病组的MPXI值(4.03±6.33)较正常对照组(-0.48±2.49)显著升高(P〈0.05);严重感染组的MPXI值较正常对熙组升高,但差异无统计学意义(P〉0.05)。同一标本的MPXI与其相应血涂片POX染色的结果呈正相关(r=0.758,P〈O.05)。结论MPXI对急性白血病的鉴别诊断具有早期提示作用。 Objective To study the correlation of the myeloperoxidase index (MPXI) and the result of blood smear peroxi- dase (POX) staining,and whether MPXI could play a role in diagnosis of leukemia. Methods MPXI of the normal(126 cases ),acute non-lymphoblastic leukemia(100 cases),acute lymphoblastic leukemia(46 cases) and serious infections pa- tients (68 cases),respectively,were detected by ADVIA2120 automatic blood cell analyzer. Blood smears from 20 cases of the primary acute leukemia were stained by the POX staining and the peroxidase positive cell percentage and score were counted. Results MPXI in the patients of acute non-lymphoblastic leukemia were significantly elevated (10.27± 12.80) compared to negative normal group(-0.48± 2.49) (P〈0. 01). The MPXI in acute lymphoblastic leukemia group were significantly increased(4.03±6.33) compared to negative normal group(-0. 48±2.49) (P〈0. 05);the MPXI was posi- tively correlated with the POX staining in the same patient (r= 0. 758,P〈0. 05). Conclusion The MPXI had some corre- lation with the result of blood smears by POX staining;MPXI has played a important role on diagnosis of acute leukemia.
