摘要 目的探讨IL-8、CRP、中性粒细胞在COPD不同分期中的变化及作用。方法将患者分为AECOPD组、S-COPD组。各组呼吸困难分级,测定FEV1%、痰液中性粒细胞、IL-8、CRP。结果(1)AECOPD组FEV1%低于稳定组,均小于70%;呼吸困难分级记分高于S-COPD组。(2)AECOPD组痰液IL-8、中性粒细胞、CRP均高于S-COPD。(3)AECOPD CRP、中性粒细胞、IL-8与呼吸困难分级记分正相关,与FEV1%负相关。结论IL-8、CRP、中性粒细胞在COPD不同分期中有显著变化,并可能参与COPD全身炎症反应和气道炎症的发生和发展的全过程,对COPD诊断与分期有一定指导意义。 Objective To investigate the changes and effects of interleukin-8(IL-8),C-reactive protein(CRP) and neutrophils in the patients with different stages of chronic obstructive pulmonary disease ( COPD ). Methods Patients were divided into AECOPD group and S-COPD group. Dyspnoeic grading was carried out in both groups,and FEV1% ,sputamentum neutrophils,IL-8 and CRP were determined. Results 1.In the AECOPD group,FEV1% was lower than that of the stable group,smaller than 70% ,and the dyspnoeic grading score was higher than that of the S-COPD group. 2.In the AECOPD group,sputamentum IL-8,neutrophils and CRP were higher than those of S-COPD. 3.In the AECOPD group, CRP, neutrophils and IL-8 were positively correlated with the dyspnoeic grading score and negatively correlated with FEV1%. Conclusion IL-8,neutrophils and CRP show marked changes in the different stages of COPD and all of them may take part in the courses of systemic inflammatory response and airway obstruction in the patients with COPD, which may guide diagnosis and staging of COPD.
