摘要
目的探讨肺癌患者的身心社会功能和生活质量情况及其影响因素。方法参照欧洲癌症治疗研究组织的生存质量量表中文版并参考肺癌相关文献自行设计肺癌患者生活质量调查问卷,经由流行病学专家审评并经2次预实验后适当修改而成,包括功能性题组、症状评分题和整体健康状态生活质量题。采用此问卷调查56例确诊的肺癌患者,对影响患者生活质量的因素进行多元线性回归分析。结果肺癌患者生活质量问卷中,功能性题组得分最高为认知功能,最低为社会功能;在症状题组中,以咳嗽咳痰症状最重,咯血症状最轻;单独题中,失眠症状最重,便秘症状最轻;整体健康状态生活质量为(46.87±19.86)分。单因素分析显示:患者的年龄、文化程度、家庭收入、身体功能状态对生活质量的影响有统计学意义;多元回归分析显示整体生活质量的影响因素为年龄和抑郁状态。结论肺癌患者在整个治疗康复过程中身心社会功能和整体生活质量均有不同程度降低。医务人员应根据评估和预测结果制定临床与居家照顾的整体方案,需要医护人员、亲友及社会的多方支持,帮助患者正确对待疾病,保持心情舒畅,增进患者在治疗和康复过程中的生活质量。
Objective To explore the status quo of the physical and psychological social functions and life quality of lung cancer patients and the influencing factors. Methods The questionnaire on the life quality of cancer lung patients was designed, based on the life quality scale by European cancer treatment and research organization and other related literature, and modified by epidemics experts before being put to application. By means of this questionnaire, 56 cases with conclusively diagnosed with lung cancer were surveyed, and a multiple regressive analysis was made of the obtained data. Results Of the questions concerning the functions, cognitive function scored highest, and social function lowest; of those concerning the symptoms, coughing and expectoration were the most severe, and hemoptysis the least; of the separate questions, insomnia were the most severe, and constipation the least. The score for the overall healthy quality of life registered （46.87±19.86）. The single factor analysis showed that the influences of the patient＇s age, educational background, family income and physical function state was statistically significant. And the multiple regressive analysis revealed that the factors influencing the overall quality of life included age and depression status. Conclusion In the whole course of treatment and rehabilitation of lung cancer patients, their physical and psychological function and quality of life all decline to various degrees. The medical staff should formulate an integrated program in light of the assessment and prediction results. The improvement of the life quality of lung cancer patients calls for the joint efforts by the medical staff, their relatives and friends and the society.
