腺苷负荷超声心动图诊断冠心病的临床评价 被引量:2

The diagnosis of coronary artery disease by adenosine stress echocardiography

摘要 目的:以冠状动脉造影(CAG)为金标准,评价腺苷负荷超声心动图(ASE)试验诊断冠心病(CHD)的敏感性和特异性。方法:40例临床疑诊CHD患者,分别静脉注射腺苷(140μg·kg-1.min-1)6min(总剂量为0.8mg/kg)。出现节段性室壁运动异常为阳性。所有患者1周内行CAG检查。结果:ASE诊断CHD的敏感性为82.6%,特异性为88.2%,准确性为85.0%;诊断单支、双支和3支冠状动脉病变的敏感性分别为75.0%,85.7%,100%,且不良反应少。结论:ASE诊断CHD安全可靠。 Objective: To assess the value of adenosine stress echocardiography in diagnosis of coronary artery disease(CAD) and to determine the sensitivity and specificity according to CAG. Method: Adenosine was injected in 40 suspectable patients with coronary artery disease with total dose of 140 μg · min ^-1 · kg^-1 in 6 rain. Positive criterion was defined as the appearance of regional wall motion abnormality. CAG was performed within 1 week. Result: The sensitivity of adenosine stress echocardiography was 82.6 %, the specificity was 88.2 %, the accuracy was 85.0%. The sensitivity for detection of myocardial isehemia in one, two and three-criminal vessel were 75.0%, 85.7%, 100% (4/4), respectively. Side effects due to adenosine infusion were always minimal. Conclusion: Adenosine stress echocardiography can be safely performed in suspectable CAD patients with high sensitivity and specificity.
