摘要
目的探讨两种剖宫产术式对二次剖宫产的影响。方法对2004年1月至2007年12月期间二次剖宫产病例进行回顾性分析。58例首次术式为新式剖宫产的再次妊娠妇女作为观察组,56例首次术式为传统子宫下段剖宫产术再次妊娠妇女作为对照组,对两组二次手术开腹时间、手术时间、术中失血量、腹腔粘连程度、子宫切口愈合与术后病率及术后肛门排气时间进行比较。结果腹腔粘连程度观察组重于对照组(P<0.05),开腹时间、手术时间、术后排气时间观察组长于对照组(P<0.05),术中失血量观察组多于对照组(P<0.05),术中子宫切口愈合及术后病率两组差异无统计学意义。结论新式剖宫产二次手术腹腔粘连严重,术中失血量多,手术时间长,术后恢复慢,对很有可能二次开腹行盆腔手术或剖宫产的病人应该慎重选择第一次手术方式。
Objective To discuss the affections of two operational methods of cesarean section on subsequent pregnancy. Methods A retrospective analysis was made to the cases of second cesarean section from Jan. , 2004 to Dec. , 2007 ; observation group included 58 cases of subsequent pregnancy, the operational method of their first cesarean section was the new style ; control group included 56 cases of subsequent pregnancy, the operational method of their first cesarean section was the traditional low segment cesarean section; the exposure time, the operation duration and the blood loss of the second section, abdominal adhesion, the healing of uterus incision, post - operation occurrence of complications and the time of post - surgery anus exhausting of the cases in both group were compared. Results Abdominal adhesion of the cases in observation group was more serious than that in control group （ P 〈 0. 05 ） ; the exposure time operation duration, the time of post - surgery anus exhausting of the cases in observation group were superior to those in control group （ P 〈 0. 05 ） ; blood loss in operation of the cases in observation group was greater than that in control group （ P 〈 0. 05 ） ; the difference between the 2 groups in the healing of uterus incision and post - operation occurrence of complications was of no statistical significance. Conclusions The second cesarean section reveals serious abdominal adhesion from the first operation, and the second operation leads to bigger blood loss, longer operation duration and slow recovery, then it should be paid attention to select the traditional operational method for those possible to have pelvic operation or cesarean section.
